IgM沉积预测原发性IgA肾病患者预后风险性及相关危险因素分析 被引量:4

摘要 目的探讨IgM沉积预测IgA肾病患者预后及在相关危险因素中的作用。方法抽取经皮肾活检确诊的IgA肾病患者90例,根据是否发生随访终点事件分为预后不良组和预后良好组。绘制Kaplan-Meier生存曲线评估IgA肾病患者的生存情况,将可能影响IgA肾病患者预后的因素进行单因素分析及多因素Cox比例风险回归模型分析。结果共有84例IgA肾病患者最终纳入该研究,预后良好组48例,预后不良组36例。IgA肾病患者存活率3个月为78%(70/90),5个月为68%(61/90),6个月为62%(56/90),8个月为59%(53/90)。单因素Cox比例风险回归模型中,肾活检前病程(P=0.043)、24 h尿蛋白量(P=0.001)、eGFR(P=0.028)、IgM沉积(P=0.001)、IgG(P=0.023)沉积对该病预后均有影响(P均<0.05)。多因素Cox比例风险回归模型显示,24 h尿蛋白量(P=0.024)、IgM沉积(P=0.001)、IgG沉积(P=0.032)是影响预后的独立影响因素;根据RR值,对患者预后的影响从大到小依次是IgM沉积(RR=2.52)、24 h尿蛋白量(RR=2.31)、IgG沉积(RR=1.52)。结论IgM沉积、24 h尿蛋白量、IgG沉积是IgA肾病患者影响预后的独立影响因素,其中IgM沉积对IgA肾病患者影响程度更大。
