C反应蛋白和血清钙水平与急性高甘油三酯血症性胰腺炎病情严重程度关系的研究

Study on the Relationship between C-reactive Protein and Serum Calcium Levels and the Severity of Acute Hypertriglyceridemic Pancreatitis

目的:探究C反应蛋白(CRP)和血清钙水平与急性高甘油三酯血症性胰腺炎(AHTGP)病情严重程度的关系。方法:回顾性分析我院104例AHTGP患者,根据亚特兰大新分类标准进行病情严重程度分级。比较不同AHTGP病情严重程度患者入院时CRP和血清钙水平的差异,行Spearman相关性分析,受试者工作特征曲线(ROC)用于评价指标的诊断效能。结果:轻症急性胰腺炎(MAP)61例,中度重症急性胰腺炎(MSAP)29例,重症急性胰腺炎(SAP)14例。MAP、MSAP和SAP患者入院时CRP水平中位数值分别为8.0,46.8和149.5 mg·mL-1(P<0.001);入院时血清钙水平中位数值分别为2.20,2.11和2.04 mmol·L-1(P=0.038)。Spearman相关性分析结果显示入院时CRP水平与AHTGP严重程度各评分系统明显正相关(P<0.05),入院时血清钙水平与AHTGP病情严重程度各评分系统明显负相关(P<0.05)。入院时CRP和血清钙水平预测病情严重程度曲线下面积分别为0.854(P<0.001)和0.669(P=0.043)。结论:入院时CRP和血清钙水平在预测AHTGP病情严重程度上具有一定的临床意义,可为临床提供参考。

Objective:To investigate the relationship between the severity of acute hypertriglyceridemic pancreatitis(AHTGP)and levels of C-reactive protein(CRP)and serum calcium.Methods:The clinical data of 104 patients with AHTGP in our hospital were enrolled retrospectively.The levels of CRP and serum calcium of different severity of AHTGP were compared.Spearmann correlation analysis was performed,and receiver operating characteristic curve(ROC)was used to evaluate the diagnostic efficacy of each indicator.Results:104 patients with AHTGP were enrolled and divided into three groups for comparative analysis:61 patients with mild acute pancreatitis(MAP),29 patients with moderately severe acute pancreatitis(MSAP),and 14 patients with severe acute pancreatitis(SAP).The median findings for the MAP,MSAP,and SAP groups were as follows:CRP after admission,8.0,46.8 and 149.5 mg·L-1 respectively(P<0.001),serum calcium after admission,2.20,2.11 and 2.04 mmol·L-1,respectively(P=0.038).Spearman’s correlation analysis showed significantly positive correlation between CRP after admission level and most acute pancreatitis scoring systems,and significantly negative correlation between serum calcium after admission level and most acute pancreatitis scoring systems.Receiver operating characteristic curve(ROC)showed that the areas under the curve for CRP and serum calcium after admission were 0.854(P<0.001)and 0.669(P=0.043),respectively.Conclusion:The levels of CRP and serum calcium after admission may predict the severity of AHTGP,which would play a reference role in clinical application.
