人工周期及克龄蒙两种方法对预防宫腔粘连电切术后再粘连的效果分析

摘要 目的探讨人工周期及克龄蒙两种方法对预防宫腔粘连电切术后再粘连的效果。方法选择2014年1月至2016年1月我中心收治的宫腔粘连患者60例,均给予宫腔粘连电切术治疗,随机分为对照组和观察组各30例。对照组采用克龄蒙治疗3个月,观察组采用雌激素人工周期治疗3个月,两组术后第3个月行取环术+宫腔镜检查术,比较两组的治疗效果、症状改善情况及远期预后。结果观察组的总有效率为83.33%,高于对照组的66.67%(P<0.05)。治疗后,观察组的月经恢复时间、术后阴道出血量、月经复潮时间及阴道流血时间均优于对照组(P<0.05)。观察组的宫腔积血、再次清宫、感染、周期性腹痛、月经异常发生率均低于对照组(P<0.05)。结论雌激素人工周期、克龄蒙用于宫腔粘连电切术后预防中均能获得良好的效果,但雌激素人工周期能改善患者的临床症状,获得良好的预后,值得推广应用。 Objective To explore the effect of artificial cycle and Climen in the prevention of readhesion after electric resection of intrauterine adhesions.Methods 60 cases of patients with intrauterine adhesions admitted to our center from January 2014 to January 2016 were given electric resection of intrauterine adhesions and were randomly divided into control group and observation group,with 30 cases in each group.The control group was treated with climen for 3 months,and the observation group was treated with estrogen artificial cycle for 3 months.Both groups were given ring removal and hysteroscopy at the third month after operation.The treatment effect,improvement of symptoms and long-term prognosis of the two groups were compared.Results The total effective rate of the observation group was 83.33%,higher than 66.67%of the control group(P<0.05).After treatment,the menstrual recovery time,postoperative vaginal bleeding volume,menstrual resurgence time and vaginal bleeding time were better than those of the control group(P<0.05).The incidences of intrauterine hemorrhage,re-clearing,infection,periodic abdominal pain and menstrual abnormalities of the observation group were lower than those of the control group(P<0.05).Conclusions Estrogen artificial cycle and climen in the prevention of readhesion after electric resection of intrauterine adhesions have good effects,but the estrogen artificial cycle can improve the clinical symptoms of patients,obtain good prognosis,and is worthy of promotion and application.
