早期脑室腹腔分流联合颅骨修补术治疗脑外伤的临床效果观察 被引量:7

Observation on Clinical Effect of Early Ventriculoperitoneal Shunt Combined with Cranioplasty in the Treatment of Brain Trauma

摘要 目的探讨早期脑室腹腔分流联合颅骨修补术治疗脑外伤的临床效果。方法选择2017年2月至2018年2月我院收治的脑外伤患者108例,随机分为两组各54例。对照组予以分期脑室腹腔分流联合颅骨修补术,观察组实施早期脑室腹腔分流联合颅骨修补术,比较两组患者的神经功能、日常生活能力、预后改善效果及并发症发生情况。结果与术前相比,两组患者术后的NIHSS评分均降低,ADL评分均升高,且观察组的NIHSS评分与ADL评分均优于对照组(P均<0.05)。观察组的预后改善效果优于对照组,并发症发生率显著低于对照组(P均<0.05)。结论早期脑室腹腔分流联合颅骨修补术能够提高脑外伤患者的日常生活能力,降低神经功能缺损程度及术后并发症发生率,有利于更好地改善预后。 Objective To explore the clinical effect of early ventriculoperitoneal shunt combined with cranioplasty in the treatment of brain trauma.Methods 108 patients with brain trauma admitted to our hospital from February 2017 to February 2018 were selected and randomly divided into two groups,with 54 cases in each group.The control group received staged ventriculoperitoneal shunt combined with cranioplasty,while the observation group received early ventriculoperitoneal shunt combined with cranioplasty.The neurological function,quality of daily life,prognosis improvement effect and complications were compared between the two groups.Results Compared with those before operation,the NIHSS scores of both groups after operation decreased,the ADL scores increased,and the NIHSS score and ADL score of the observation group were better than those of the control group(all P<0.05).The prognosis improvement effect of the observation group was better than that of the control group,and the incidence of complications was lower than that of the control group(both P<0.05).Conclusions Early ventriculoperitoneal shunt combined with cranioplasty can improve the quality of daily life in patients with brain trauma,reduce the neurological impairment degree and the incidence of postoperative complications,and help to better promote prognosis.
