卒中后肢体痉挛针刺治疗研究进展

Research progress on acupuncture therapy for post-stroke limb spasm

肢体痉挛是卒中后常见并发症,卒中后肢体痉挛的发生减少了卒中患者的移动,不利于卒中患者的康复成功。针刺以其简、便、廉、验特点在卒中后肢体痉挛的治疗中有着重要作用,目前常见的治疗卒中后肢体痉挛选穴方案主要包括夹脊穴法、平衡肌张力针法、针刺拮抗肌群穴法和经筋刺法等。其中后3个方案选穴均在痉挛局部,但现代医学对痉挛局部选穴存在较大争议,且缺乏随机、对照、盲法等高质量研究设计。夹脊穴避开了肢体痉挛局部取穴争论的焦点,且王氏夹脊穴取穴少,简单易操作,无不良反应,对于降低痉挛严重程度,促进患者肢体运动功能康复具有重要意义。

Limb spasm is a common complication after stroke. The occurrence of post-stroke spasm reduces the movement of stroke patients, which is not conducive to the success of rehabilitation of stroke patients. Acupuncture plays an important role in the treatment of post-stroke spasm because of its simplicity, convenience, inexpensiveness and experience. At present, the common acupoint selection schemes for post-stroke spasm mainly include Jiaji acupoint method, balance muscle tension needle method, acupuncture at antagonistic muscle group acupoints method and meridian and tendon acupuncture method, etc. Among them, the latter three schemes are all in the spastic area, but there is much controversy in modern medicine about the spastic local selection, and there is a lack of randomized, controlled, blind and other high-quality research design. WANGs’ Jiaji acupoints avoid the focus of controversy on local acupoint of limb spasm, have the less number of acupoints, simple and easily operation, less adverse reactions, are very significant to reduce the severity of limb spasm and promoting the rehabilitation of motor function.
