正常甲状旁腺及甲状旁腺功能亢进超声影像学特征分析

Ultrasonographic characteristics of normal parathyroidism and hyperparathyroidism

目的探讨正常甲状旁腺及甲状旁腺功能亢进的超声影像学特征。方法回顾性分析自2012年9月至2018年11月福建医科大学附属漳州市医院收治的8例原发性甲状旁腺功能亢进(PHPT)患者、17例继发性甲状旁腺功能亢进(SHPT)患者、117例甲状旁腺正常的受检者的甲状旁腺影像特征资料。结果甲状旁腺正常的受检者下组的检出率为61.5%(72/117),显著高于上组的17.9%(21/117),差异有统计学意义(P<0.05)。8例PHPT和17例SHPT的患者分别检出14枚和51枚甲状旁腺。正常甲状旁腺、PHPT、SHPT在回声、有无钙化及血流特征方面比较,差异有统计学意义(P<0.05)。结论超声检查可以显示正常甲状旁腺,其下组检出率较上组检出率高。甲状旁腺功能亢进时,甲状旁腺呈低回声、血供相对丰富。

Objective To investigate the Ultrasonographic features of normal parathyroid gland and hyperparathyroidism.Methods A retrospective study was performed on 8 cases of patients with primary hyperparathyroidism(PHPT),17 cases of patients with secondary hyperparathyroidism(SHPT)and 117 cases of subjects with normal parathyroid glands from September 2012 to November 2018.Results The detection rate of the lower group with normal parathyroid glands was 61.5%(72/117),significantly higher than the 17.9%(21/117)of the upper group,and the difference was statistically significant(P<0.05).The 14 and 51 parathyroid glands were detected in 8 patients with PHPT and 17 patients with SHPT,respectively.Comparison of normal parathyroid glands,PHPT and SHPT in echogenicity,calcification and blood flow characteristics showed statistically significant differences(P<0.05).Conclusion Ultrasonography showed normal parathyroid glands,and the detection rate of the lower group was higher than that of the upper group.In hyperparathyroidism,the parathyroid gland presents hypoechoic and relatively rich blood supply.
