开郁祛痰活血汤治疗冠心病心绞痛合并抑郁症

Kaiyu Qutan Huoxue Decoctionin Treating Angina Pectoris of Coronary Heart Disease Combined with Depression

目的观察开郁祛痰活血汤治疗冠心病心绞痛合并抑郁症的有效性和安全性。方法将152例冠心病心绞痛合并抑郁症患者随机分为观察组和对照组,各76例。对照组给予硝酸酯类、抗血小板类、SSRI类等西药治疗,观察组在此基础上加用开郁祛痰活血汤口服。治疗时间8周。结果观察组疾病疗效和证候积分疗效的临床总有效率分别为86.8%、93.4%,均优于对照组的70.3%、77.0%(均P <0.05);观察组中医证候总积分、汉密尔顿抑郁量表评分均大幅下降,心绞痛的发作次数、持续时间、疼痛程度、硝酸甘油用量均明显减少,与对照组相比,均达到显著统计学差异(均P <0.01)。研究过程中未发生不良事件。结论开郁祛痰活血汤能够改善冠心病心绞痛合并抑郁症患者的心绞痛证候和抑郁状态。

Objective To observe the efficacy and safety of Kaiyu Qutan Huoxue decoction in treating angina pectoris of coronary heart disease with depression. Methods 152 patients with angina pectoris of coronary heart disease with depression were randomly divided into observation group and control group, 76 cases in each group. The control group was treated with standard western medicine, while the observation group was treated with Kaiyu Qutan Huoxue decoction on the basis of Western medicine. The treatment lasted for 8 weeks. Results The total clinical effective rates in effect of disease and syndrome integral therapy of the observation group were 86.8% and 93.4% respectively, which were better than 70.3% and 77.0% of the control group(all P < 0.05). The total scores of TCM syndromes and Hamilton Depression Scale in the observation group decreased significantly, and the number of attacks, duration(minutes), pain degree and nitroglycerin dosage(tablets) of angina pectoris decreased significantly compared with the control group(all P < 0.01). No adverse events occurred during the test. Conclusion Kaiyu Qutan Huoxue decoction has definite clinical effect and low safety risk in the treatment of angina pectoris of coronary heart disease with depression.
