上消化道出血中应用生长抑素对止血效果的影响

Effect of Somatostatin on Hemostasis in Upper Gastrointestinal Hemorrhage

目的探讨上消化道出血中应用生长抑素对止血效果的影响。方法随机抽样法选取2018年1月至2019年1月本院114例上消化道出血患者。随机数字表法分组:对照组(57例)采用常规治疗,研究组(57例)在常规治疗基础上加用生长抑素。比较两组止血效果。结果研究组即刻止血成功率、总体止血有效率均高于对照组(χ^2=4.398、5.479,P=0.035、0.019),72 h再出血率低于对照组(χ^2=4.342,P=0.037),呕血消失时间短于对照组(t=4.728,P=0.000),大便潜血试验转阴时间短于对照组(t=19.116,P=0.000);研究组不良事件发生率为5.26%,对照组则为7.02%(χ^2=0.000,P=1.000)。结论上消化道出血治疗中加用生长抑素可改善止血效果,降低再出血率,且不良事件未见增加。

Objective To investigate the effect of somatostatin on hemostasis in upper gastrointestinal hemorrhage.Methods 114 patients with upper gastrointestinal hemorrhage in our hospital from January 2018 to January 2019 were selected with random sampling method and grouped by random number table:the control group(57 cases)received conventional treatment,while the study group(57 cases)was treated with somatostatin in addition to conventional therapy.And the hemostatic effects of the two groups were compared.Conclusion The effective rate of hemostasis,overall hemostasis efficiency in the study group was higher than that in the control group(χ^2=4.398、5.479,P=0.035、0.019)and the rehaemorrhage ratio after 72 hours was lower than that in the control group(χ^2=4.342,P=0.037),the disappearance time of hematemesis was shorter than that of the control group(t=4.728,P=0.000)and the time for stool occult blood test to turn negative was shorter than that of the control group(t=19.116,P=0.000).The incidence of adverse events was 5.26%in study group and it was 7.02%in control group(χ^2=0.000,P=1.000).Conclusion In the treatment of upper gastrointestinal hemorrhage,somatostatin can improve the hemostasis effect,reduce the rate of re-bleeding,and reduce the number of adverse events.
