贵阳市城区40~79岁代谢综合征人群发生心血管事件和各种死亡风险的前瞻性队列研究 被引量:9

A prospective study on the influence of metabolic syndrome on the risks of cardiovascular disease events and all-cause mortality for urban residents aged 40-79 years in Guiyang

摘要 目的探讨代谢综合征人群发生心血管事件和各种死亡的风险。方法采用前瞻性队列研究,对贵阳市40~79岁城区居民平均随访3年,随访终点事件为心血管事件和各种死亡,利用COX比例风险模型分析有无代谢综合征发生终点事件的HR值及其95%CI。代谢综合征诊断按照2005年国际糖尿病联盟(IDF)定义。结果共纳入研究人群7313名,发生终点事件226例,其中心血管事件146例,死亡80例,调整性别、年龄、吸烟、饮酒、血脂危险因素后,代谢综合征者与非代谢综合征者比较,心血管事件发病和各种死亡风险HR=1.43(P=0.037)、HR=1.25(P=0.418);代谢综合征者较无代谢综合征组分者发生心血管事件风险增加1.43倍(P=0.013),各种死亡风险无显著增加。结论代谢综合征者心血管事件发病风险高于非代谢综合征者,各种死亡风险无显著增加。代谢综合征是心血管事件发病的重要危险因素。 Objective To explore the influence of metabolic syndrome on the risks of cardiovascular disease (CVD) events and all-cause mortality.Methods In this prospective cohort study, urban residents aged 40-79 years in Guiyang were followed-up for three years. The end-points were CVD events and all-cause mortality. COX proportional hazards model were used for the corresponding hazard ratios (HRs) of CVD events and all-cause mortality. Metabolic syndrome was defined according to the 2005 International Diabetes Federation (IDF) criteria.Results A total of 7313 subjects were included. 146 cases of CVD events and 80 cases of all-cause mortality were recorded. After adjusting for gender, age, smoking, alcohol consumption, and blood lipid levels, the metabolic syndrome increased the risks of CVD events and all-cause mortality. The HRs were 1.43 (P=0.037) and 1.25 (P=0.418), respectively. As compared metabolic syndrome with non-metabolic syndrome, an increased risk of CVD events was found. The HR was 1.43 (P=0.013). No significantly increased risk of all-cause mortality was found in subjects with metabolic syndrome.Conclusion The metabolic syndrome was associated with increased risk of CVD events and there was no significant increase in all-cause mortality. Metabolic syndrome is an important risk factor for CVD events.
