医药价格改革前后珠海市公立医院收入结构变化分析

Analysis on the change of income structure of public hospitals before and after the Reform of pharmaceutical price in Zhuhai city

[目的]分析医药价格改革前后珠海市公立医院收入结构变化,为建立科学合理的控费机制提供参考。[方法]采用描述性统计分析方法对全市公立医院改革前后2年服务量及收入数据进行同期对比分析。[结果]珠海市公立医院在改革前的总诊疗人次为1294.3万人次、出院人次为38.3万人次、平均住院日为8.68 d、病床使用率为84.62%;改革后的总诊疗人次为1399.7万人次、出院人次为47.8万人次、平均住院日为8.31 d、病床使用率为86.30%。药品收入占比从2013年的42.35%下降到2016年的31.56%,医务收入占比和检查化验收入占比从2013年的22.45%和25.54%%增加到26.80%和31.33%。[结论]改革后,珠海公立医院医疗服务量稳步增长,收入结构趋于合理;改革对药费控制效果明显,但对整体医疗费用增长作用有限。

ObjectiveThe aim of this study is to analyze changes of public hospitals income structurebefore and after the reform of pharmaceutical prices in Zhuhai city so as to provide reference for setting up scientific and reasonable fee control mechanism.Methods It conducted contrastive analysis at the corresponding period to service amount before and after 2 years reform of public hospitals in the city and the income data by descriptive statistical analysis.Results Before the reform,total diagnosis and treat person-time was 12.943 million,383 thousand person-time discharged,average length of stay was 8.68 days,rate of utilization of hospital beds was 84.62%.After the reform,total diagnosis and treat person-time was 13.997 million,478 thousand person-timedischarged,average length of stay was 8.83 days,rate of utilization of hospital beds was 86.30%.The income proportion of medicines from 42.35%in 2013 decreased to 31.56%in 2016.The income proportion of medical matters,inspection and test were from 22.45%and 25.54%increased to 26.80%and 31.33%.Conclusions After the reform,medical service amount is in steady-state growth,hospital income structure tends to be reasonable.The reform has obvious effects on drug control,but it has limitation on overall medical expenses.
