577nm与532nm激光在非增生型糖尿病视网膜病变中全视网膜激光光凝治疗的体会 被引量:1

摘要 目的:对比观察577nm、532 nm波长激光在非增生型糖尿病视网膜病变(NPDR)中全视网膜激光光凝(PRP)治疗效果。方法:收集2017年1月到2019年6月就诊于我院的临床确诊的重度NPDR患者33例58只眼,随机数字表法将患眼分为577 nm组16例28只眼和532 nm组17例30只眼。分别使用577 nm单点模式加矩阵模式、532 nm激光单点模式行PRP。随访3-6月,记录两组激光功率、光斑点数、爆破时间、完成PRP次数、每次治疗持续时间、患者耐受情况及治疗效果。结果:完成标准PRP,三级光斑,577 nm组激光次数3次、光斑直径200μm、爆破时间20ms,激光能量500-800mw、激光点数700-800次、视力提高明显,每次激光时间短,患者耐受较好。随诊6月,视网膜色素增殖轻,视网膜再发新生血管3只眼,给予补充激光治疗。532 nm组激光次数4-5次、爆破时间150ms,激光能量200-400mw、激光点数400-500次、视力提高,每次激光时间较长,患者耐受差。随诊6月,视网膜色素增殖重,视网膜再发新生血管6只眼,给予补充激光,组间差距比较包含统计学意义(P<0.05)。结论:对于NPDR患者行PRP,患者采用577nm激光比532nm激光治疗效果好、治疗周期短、治疗时疼痛较轻,患者耐受较好,爆破时间短、视网膜损伤轻,视网膜色素增殖不重,视力提高较好,值得临床广泛运用。
