摘要
目的:探究分析前列腺癌Gleason评分与动态增强磁共振成像(DCE-MRI)的相关性。方法:回顾性选取医院收治的经病理活检证实的67例前列腺癌患者病例资料,获取其DCE-MRI定量参数,根据Gleason评分将67例患者中Gleason评分≤6分纳入低危组(19例),Gleason评分>6分纳入中高危组(48例),分析前列腺癌Gleason评分与动态增强磁共振的相关性。结果:低危组与中高危组患者3.0 T DCE-MRI定量参数直方图参数(25%位数、中位数、75%位数、平均值)中速率常数(Kep)值、转运常数(Ktrans)值对比差异有统计学意义[Kep(t=9.261,t=7.765,t=6.861,t=8.490;P<0.05),K^trans(t=3.457,t=3.477,t=1.482,t=3.676;P<0.05)];而两组直方图参数血管外细胞外间隙体积百分数(Ve)值比较差异无统计学意义(P>0.05);3.0 T DCE-MRI定量参数中K^trans、Kep与病理Gleason评分(25%位数、中位数、75%位数、平均值)呈正相关[K^trans(r=0.485,r=0.430,r=0.365,r=0.428;P<0.05),Kep(r=0.675,r=0.661,r=0.597,r=0.693;P<0.05)]。结论:前列腺癌Gleason评分与DCE-MRI定量参数呈线性相关,可对病变分级信息准确反映,有助于对前列腺癌的恶性程度进行初步判断。
Objective:To explore the correlation between Gleason score of patients with prostatic cancer and 3.0 T dynamic contrast enhanced magnetic resonance imaging(3.0 T DCE-MRI)of them.Methods:The data of 67 patients with prostatic cancer who were confirmed by pathological biopsy were retrospectively selected.The quantitative parameters of 3.0 T DCE-MRI of them were obtained.According to Gleason score,67 patients were divided into lowrisk group(19 cases,Gleason score≤6 points)and middle-high-risk group(48 cases,Gleason score >6 points).And then,the correlation between Gleason score of prostatic cancer and DCE-MRI was analyzed.Results:The differences of Kepand K^trans in histogram parameters(P25,median,P75 and mean value)of quantitative parameters of 3.0 T DCEMRI between low-risk group and middle-high-risk group were significant[Kep(t=9.261,t=7.765,t=6.861,t=8.490,P0.05),K^trans(t=3.457,t=3.477,t=1.482,t=3.676,P<0.05)],while the volume percentage(Ve)of extracellular space of histogram parameters between two groups was no significant(P>0.05).In quantitative parameters of 3.0 T DCEMRI,K^trans and Kep were positively correlated with pathological Gleason score(P25,median,P75 and mean value)[K^trans(r=0.485,r=0.430,r=0.365,r=0.428,P<0.05),Kep(r=0.675,r=0.661,r=0.597,r=0.693,P<0.05)],respectively.Conclusion:The Gleason score of prostatic cancer is linearly correlated with the quantitative parameters of 3.0 T DCEMRI.And it can accurately reflect the grading information of lesion and contribute to primarily judge the malignant degree of prostate cancer.
