摘要
目的:探讨肺超声及肺超声评分在新生儿肺部感染严重程度的应用价值。方法:回顾性选取86例肺部感染新生患儿,将呼吸衰竭患儿纳入观察组(46例),无呼吸衰竭患儿纳入对照组(40例)。86例新生儿均给予超声检查,分析超声图像,并对患儿进行肺超声评分,统计各临床指标,分析超声评分与危重患儿评分系统(NCIS)评分、呼吸指数(RI)及氧合指数(OI)的相关性。结果:肺部感染患儿肺部超声表现多为肺实变、肺泡间质综合征、胸膜线异常与局限性胸腔积液等。观察组患儿肺超声评分和OI低于对照组,RI高于对照组,差异有统计学意义(t=13.396,t=11.186,t=16.632;P<0.001);观察组中随着患儿的危重程度越重,其NCIS评分、超声评分及OI指数越低,RI指数越高,差异具有统计学意义(F=8.354,F=16.324,F=14.286,F=9.874;P<0.001)。观察组患儿肺超声评分与NCIS评分呈正相关(r=0.796,P<0.05),与OI呈正相关(r=0.779,P<0.05),与RI呈负相关(r=-0.799,P<0.05)。结论:肺超声评分可以用于评估新生儿肺部感染患儿的严重程度,对患儿的尽早诊治有重要意义,应用价值较高。
Objective:To investigate the application value of pulmonary ultrasound and its score in the severity of newborns with pulmonary infection.Methods:86 newborns with pulmonary infection were retrospectively selected.According to the situation of respiratory failure,46 cases were divided into observation group(newborns with respiratory failure,n=46)and control group(newborns without respiratory failure,n=40).All of 86 newborns were examined by ultrasound.Their ultrasonic images were analyzed,and the pulmonary ultrasound scores were implemented for them,and each clinical indexes of them were used to implement statistical analysis.And the correlations between ultrasound score and neonatal critical illness score(NCIS)score,and between ultrasound score and respiratory index(RI),and between ultrasound score and oxygenation index(OI)were further analyzed.Results:Pulmonary consolidation,alveolar interstitial syndrome,pleural line abnormality and localized pleural effusion were most manifestations in newborns with pulmonary infection.The pulmonary ultrasound score and OI of observation group were significantly lower than those of control group,while RI of observation group was significantly higher than that of control group(t=13.396,t=11.186,t=16.632,P<0.001).In the observation group,the NCIS score,the ultrasonic score,the OI was lower and lower,and the RI was higher and higher with the increasing of severity of newborns with pulmonary infection,and the differences were statistically significant(F=8.354,F=16.324,F=14.286,F=9.874,P<0.001).And in observation group,Pearman correlation analysis showed that there was a significantly positive correlation between pulmonary ultrasound score and NCIS score(r=0.796,P<0.05).And it was a significantly positive correlation between pulmonary ultrasound score and OI(r=0.779,P<0.05).While it was a significantly negative correlation between pulmonary ultrasound score and RI(r=-0.799,P<0.05).Conclusion:The pulmonary ultrasound score can be used to assess the severity of newborns with pulmonary infection,and it has great significance for the early diagnosis and treatment of newborns with pulmonary infection.Therefore,it has higher application value.
