右正中神经电刺激在弥漫性轴索损伤治疗中的应用效果 被引量:7

摘要 目的探讨右正中神经穴位针灸合并电刺激治疗在颅脑弥漫性轴索损伤治疗中的应用效果。方法将60例中、重度弥漫性轴索损伤患者按随机数字表法分为治疗组和对照组,每组30例。对照组给予降颅压,营养神经,预防感染,防治并发症,维持水、电解质平衡和支持对症,康复训练等常规治疗;治疗组在常规治疗基础上给予右正中神经穴位针灸合并低频电刺激进行治疗。比较2组治疗前后格拉斯哥昏迷量表(GCS)评分变化情况及临床疗效。结果2组治疗前GCS评分比较差异无统计学意义(P>0.05);2组治疗后GCS评分均较治疗前显著升高(P<0.05),且治疗组较对照组升高更为显著[(10.53±3.82)分比(8.72±2.27)分,P<0.05]。治疗组临床疗效优良率较对照组显著升高(53.3%比33.3%,P<0.05)。结论正中神经电刺激可有效促进弥漫性轴索损伤患者神经功能恢复,改善患者意识状态,减轻残疾程度。 Objective To investigate the efficacy of acupuncture combined with electrical stimulation at right median nerve in the treatment of diffuse axonal injury.Methods Sixty patients with moderate to severe diffuse axonal injury were randomly divided into treatment group and control group,with 30 patients in each group.Both groups received routine treatment,including intracranial pressure reduction,neurotrophy,prevention of infection,prevention and treatment of complications,maintenance of water and electrolyte balance,symptomatic treatment,and rehabilitation training.Additionally,the treatment group was given acupuncture combined with low-frequency electrical stimulation at right median nerve.The Glasgow Coma Scale(GCS)score and clinical efficacy were compared between the two groups.Results There was no significant difference in GCS score between the two groups before treatment(P>0.05).After treatment,GCS score increased in both groups,and the score in treatment group(10.53±3.82)was higher than that in control group(8.72±2.27)(P<0.05).Moreover,the excellent and good rate of clinical efficacy in treatment group(53.3%)was higher than that in control group(33.3%)(P<0.05).Conclusion Right median nerve electrical stimulation can effectively promote nerve function recovery,improve consciousness and reduce disability in patients with diffuse axonal injury.
