中老年膝骨关节炎患者临床症状与焦虑的相关性研究

Correlation between Clinical Symptoms and Anxiety in the Middle and Old Aged with Knee Osteoarthritis

目的探讨中老年膝骨关节炎(Knee Osteoarthritis,KOA)患者的焦虑与膝骨关节炎临床症状的相关性。方法2017年11月-2018年6月于曙光医院骨伤科住院部、门诊招募219例膝骨关节炎患者年龄范围为41~93岁,平均年龄为(63.82±11.45)岁,采集基本情况(性别,婚姻状况,伴随病,居住方式,身高,体重,计算体质指数等)、医院焦虑抑郁情绪测量表(HADS)、膝骨关节炎评分量表(WOMAC)。以EpiDate3.0软件录入与管理数据,SPSS17.0软件统计分析,将焦虑与膝骨关节炎24种临床症状进行偏相关分析。结果中老年膝骨关节炎患者中焦虑的病例为141例(64.38%),KOA患者焦虑评分高于上海地区常模水平,差异存在统计学意义(P<0.05);情绪和临床症状相关性方面,焦虑与24种临床症状均存在统计学意义(P<0.05),其中与脱袜及走出浴缸的躯体活动限制症状的相关性较大(R=0.386,R=0.361)。结论老年膝骨关节炎患者人群中存在较高比例的焦虑;焦虑与膝骨关节炎临床症状严重程度正相关,躯体活动限制症状(脱袜及走出浴缸)与KOA患者焦虑的相关性较大。

Objective To explore the correlation between clinical symptoms and anxiety in the middle and old aged with knee osteoarthritis(KOA).Methods 219 cases with KOA were selected during the period from Nov.2017 to June.2018,aged from 41 to 93 years with the average of(63.82±11.45)years;the general information,the scores of Hospital Anxiety and Depression Scale(HADS)and the scores of McMaster Osteoarthritis Index(WOMAC)were collected;the software EpiDates3.0 was applied in data input and management while software SPSS17.0 in statistical analysis;the partial correlation between anxiety and 24 clinical symptoms of KOA was analyzed.Results Out of the 219 cases,anxiety was found in 141 cases(64.38%),the anxiety scores of KOA cases were higher than that of norms in Shanghai with statistical difference(P<0.05);there existed a statistical difference;anxiety was in certain correlation with the 24 clinical symptoms with statistical significance(P<0.05)and was in a greater correlation with the symptoms of physical activity limitation,such as sock removal and out-of-bathtub(R=0.386,R=0.361).Conclusions The proportion of anxiety occurrence is bigger in the elderly with KOA;anxiety is in positive correlation with the severity of clinical symptoms of KOA;the physical activity limitation symptoms socks removal and out-of-bathtub of the cases with KOA are in a greater correlation with anxiety.
