腹腔镜胆总管探查术治疗胆总管结石的临床研究

Clinical Study of Laparoscopic Common Bile Duct Exploration for Common Bile Duct Calculi

目的:探索腹腔镜胆总管探查术在胆总管结石患者中的应用价值。方法:选取我院普外科2015年2月—2017年12月收治的胆总管结石患者62例为研究对象,根据治疗方法分组,对照组行开腹胆总管探查术,观察组行腹腔镜胆总管探查术,收集两组的围术期指标。结果:观察组术中出血量低于对照组,P<0.05;手术时间、术后首次排气时间、术后住院时间:观察组均短于对照组,P<0.05;术后随访1年,观察组的结石复发率与对照组差异不明显,P>0.05。结论:腹腔镜胆总管探查术的应用有助于减轻胆总管结石患者的手术创伤,促进术后康复,且手术彻底,复发率低,值得推广。

Objective:To explore the value of laparoscopic common bile duct exploration in patients with common bile duct calculi.Methods:62 patients with common bile duct calculi admitted to our general surgery department from February 2015 to December 2017 were selected as the study subjects.According to the treatment methods,the control group was treated with open common bile duct exploration,while the observation group was treated with laparoscopic common bile duct exploration.Perioperative indicators of the two groups were collected.Results:The bleeding amount of observation group is lower than the control group,P<0.05;Operation time,postoperative exhaust time,postoperative hospital stay for the first time:the observation group were shorter than the control group,P<0.05;After 1 year of follow-up,the recurrence rate of stones in the observation group was not significantly different from that of the control group,P>0.05.Conclusion:The application of laparoscopic common bile duct exploration is helpful to alleviate the surgical trauma of patients with common bile duct calculi and promote the recovery after operation.The operation is thorough and the recurrence rate is low.It is worth popularizing.
