CT和MRI对膝关节半月板与关节软骨损伤诊断效能的比较 被引量:8

摘要 目的比较CT和MRI对膝关节半月板与关节软骨损伤的临床诊断效果。方法选取膝关节半月板和关节软骨损伤患者127例,其中61例半月板损伤,66例关节软骨损伤。对所有患者分别进行CT和MRI扫描诊断,以关节镜下诊断为对照标准比较两种诊断方法的准确率。结果MRI对半月板损伤诊断准确率95.08%,高于CT的83.61%,共用MRI和CT对Ⅰ、Ⅱ级半月板损伤的检测准确率差异无统计学意义(P>0.05),MRI对Ⅲ级半月板损伤诊断准确率86.67%,高于CT的46.67%,差异有统计学意义(P<0.05);对关节软骨损伤检测中,MRI的诊断准确率90.91%,高于CT的81.84%。MRI对Ⅱb期和Ⅳ级关节软骨损伤检测准确率分别为88.24%和85.71%,均高于CT检测的76.47%和42.86%,差异有统计学意义(P<0.05);两种诊断方式对Ⅰ级、Ⅱa期和Ⅲ级关节软骨损伤检测准确率差异无统计学意义(P>0.05)。结论MRI对Ⅱb期与Ⅳ级关节软骨损伤和Ⅲ级膝半月板损伤临床诊断准确率高于CT诊断。 Objective To explore the clinical diagnosis effects between CT and MRI for meniscus and cartilage articularis injure.Methods A total of 127 patients,61 patients got meniscus injured and the other 66 got cartilage articularis injured were chosen.All the patients received CT and MRI diagnosis for their diseases and the results of arthroscopy diagnosis were used as the reference standard.Results The diagnostic rate of MRI for meniscus injury was 95.08%while CT was 83.61%.MRI had higher diagnostic rate than CT.There was no significant difference between CT and MRI when diagnosis forⅠandⅡstage meniscus.The accuracy of MRI in detecting gradeⅢmeniscus injury was 86.67%,higher than CT(46.67%).The difference was statistical significance(P<0.05).The accuracy of MRI in detecting Joint soft tissue injury was 90.91%,higher than CT(81.84%).The accuracy of MRI in detecting joint soft tissue injuryⅡb andⅣwas 88.24%and 85.71%,both higher than CT(76.47%and 42.86%).There was statistical significance(P<0.05).There was no statistical significance in detecting joint soft tissue injuryⅠ,Ⅱa andⅢfor the two methods.Conclusion MRI has higher diagnostic rate for II b andⅣstage cartilage articularis injure as well asⅢstage meniscus injure than CT.
