认知护理对肾衰竭血液透析患者放疗的影响 被引量:1

摘要 目的探讨认知护理对肾衰竭血液透析患者肾功能、机体营养和生活质量的影响。方法160例肾衰竭血液透析患者随机分为观察组和对照组,各80例。对照组行肾内科血液透析常规护理,观察组在常规护理基础上,行认知护理,连续护理12周。观察2组护理前后血尿素氮(BUN)、血清肌酐(Scr)等肾功能指标和主观营养评分(SGA)、营养不良-炎性评分(MIS)以及健康调查简表(SF-36)各维度评分变化。结果护理前观察组BUN、Scr水平和SGA、MIS评分分别为(18.53±4.21)μmol/L、(342.48±49.96)mmol/L、(14.41±3.02)分及(11.24±2.93)分,对照组分别为(18.39±4.32)μmol/L、(235.64±10.45)mmol/L、(14.36±2.81)分及(11.15±3.26)分,差异无统计学意义(P>0.05);护理后,观察组BUN、Scr水平和SGA、MIS评分分别为(8.47±1.56)μmol/L、(343.62±48.75)mmol/L、(5.39±1.14)分及(4.60±1.08)分,对照组分别为(10.02±1.63)μmol/L、(288.57±20.33)mmol/L、(8.62±1.37)分及(8.17±1.24)分,差异有统计学意义(P<0.05);护理前2组SF-36各维度评分比较,差异均无统计学意义(P>0.05);护理后,观察组SF-36各维度评分为生理功能(82.19±9.03)、生理职能(19.13±4.25)、躯体疼痛(69.94±10.25)、总体健康(61.45±6.39)、生命活力(63.59±9.91)、社会职能(59.96±6.58)、情感职能(28.81±2.35)及精神健康(59.13±4.42)分,均分别高于对照组的(78.02±8.19)分、(18.35±3.94)分、(65.27±9.13)分、(56.71±6.93)、(62.35±9.87)、(52.61±5.03)、(25.15±2.53)及(53.01±3.69)分,观察组护理后SF-36量表各维度评分除生理职能和生命活力评分外,差异均有统计学意义(P<0.05)。结论认知护理可明显改善肾衰竭血液透析患者肾功能和机体营养状况,使患者生活质量得到明显提高,有助于改善预后。
