胫骨横向骨搬移术联合MEBT/MEBO治疗下肢静脉曲张术后难愈性溃疡疗效观察

Observation on the Curative Effect of Transverse Tibial Bone Transport Combined with MEBT/MEBO in the Treatment of Postoperative Refractory Ulcers of Varicose Veins of Lower Extremities

目的探讨胫骨横向骨搬移术联合烧伤创疡再生医疗技术(moist exposed burn therapy/moist exposed burn ointment,MEBT/MEBO)治疗下肢静脉曲张术后难愈性溃疡的临床疗效。方法对2015年1月至2017年3月盐城同洲骨科医院收治的18例下肢静脉曲张术后难愈性溃疡患者行胫骨横向骨搬移术联合湿润烧伤膏换药治疗,观察治疗效果。结果本组18例患者创面均完全愈合,创面愈合时间为20~30 d,固定支架拆除时间为2~3个月,治疗过程中均未出现皮肤坏死、针道感染、胫骨截骨处继发骨折及局部感染等并发症。出院后随访6~21个月,所有患者均未复发。结论胫骨横向骨搬移术联合MEBT/MEBO治疗下肢静脉曲张术后难愈性溃疡,可有效促进创面愈合,效果显著,且复发率低,值得临床推广应用。

Objective To investigate the curative effect of transverse tibial bone transport combined with moist exposed bum therapy/moist exposed burn ointment(MEBT/MEBO)in the treatment of refractory ulcers following the operation for varicose veins of lower extremities.Methods Eighteen patients with refractory ulcers following the operation for varicose veins of lower extremities admitted into Yancheng Tongzhou Orthopedic Hospital between January 2015 and March 2017 were treated with the transverse tibial bone transport combined with the dressing change of MEBO to observe the curative effect.Results The wounds of the 18 patients were all healed.The wound healing time was 20-30 d and the removal time of fixator was 2-3 months.No complication,such as skin necrosis,pin infection,secondary fracture at the location of tibial osteotomy,or local infection,was observed during the treatment course.The follow-up of 6-21 months after the discharge found no relapse in all patients.Conclusion In treating refractory ulcers following the operation for varicose veins of lower extremities,the combined application of transverse tibial bone transport and MEBT/MEBO can effectively promote wound healing with a low relapse rate,deserving to be promoted in clinical practice.
