局部注射万古霉素预防全膝关节置换术感染 被引量:4

摘要 目的研究局部注射万古霉素在膝关节置换术中预防感染的有效性。方法选取2016年1月至2019年1月在我科进行初次全膝关节置换手术的患者126例。根据围手术期预防应用抗生素的方法分为常规组和万古霉素组,比较两组患者术后的感染发生率。结果3例患者出现感染,总感染率为2.3%,其中常规组为2(4.3%),万古霉素组为1例(1.2%)。两组间感染率比较差异有统计学意义(P<0.05)。结论局部关节腔内注射万古霉素可降全膝置换术后感染率。并且万古霉素局部应用简单、可操作性强、费用低,我们建议,全膝关节置换手术的患者术中局部应用万古霉素1.0 g预防感染。 Objection To study the effectiveness of local vancomycin injection in preventing infection in total knee arthroplasty.Method 126 patients who underwent initial total knee arthroplasty in our department from January 2016 to January 2019 were selected.According to the method of using antibiotics,the patients were divided into two groups:control group and vancomycin group,the incidence of postoperative infection was compared between the two groups.Results Infection occurred in 3 patients,with a total infection rate of 2.3%,including 2 cases(4.3%)in the control group and 1 case(1.2%)in the vancomycin group.The difference in infection rate between the two groups was statistically significant(P<0.05).Conclusion Local Injection of vancomycin can reduce the infection rate after total knee arthroplasty.In addition,the topical application of vancomycin is simple,operable and low cost.Therefore,we recommend that local injection of vancomycin 1.0 g to prevent infection in patients undergoing total knee arthroplasty.
