下肢慢性静脉功能不全患者小腿部位穿支静脉内径大小对其功能的预测价值

Predictive value of internal diameter of perforator veins of lower extremities for its function in patients with chronic venous insufficiency

目的探讨下肢慢性静脉功能不全(CVI)患者小腿部位穿支静脉内径大小对其功能是否完全的预测价值。方法2017年6月至2018年12月上海交通大学附属第六人民医院收治按照CEAP分级为C4~C6的421例CVI患者(421条患肢),运用彩色多普勒超声(CDUs)检测患肢小腿部位穿支静脉功能状态。采用受试者工作特征(ROC)曲线分析CVI患者小腿部位穿支静脉内径大小对其功能是否完全的预测价值。结果穿支静脉内径作为其功能是否完全预测指标的最佳临界值为3.3 mm,预测的灵敏度和特异度分别为87%和95%。结论CVI患者下肢小腿部位穿支静脉的内径大小对其功能是否完全具有预测价值,下肢小腿部位功能不全穿支静脉的治疗对改善CVI患者的临床表现具有重要意义。

Objective To evaluate the predictive value of internal diameter of perforator veins(PVs)of lower extremities for its function in patients with chronic venous insufficiency(CVI).Methods 421 CVI patients(421 limbs)with CEAP classification of C4~C6 admitted in Shanghai Jiao Tong University Affiliate Sixth People’s Hospital between June 2017 and December 2018 were recruited.Color Doppler ultrasounds(CDUs)were used to detect the function of PVs.The receiver operating characteristic(ROC)curve was used to analyze the predictive value of the diameter of PVs for its function.Results The optimal cut-off value of the diameter of PVs as a predictor for whether its function was competent was 3.3 mm,and the predicted sensitivity and specificity were 87%and 95%,respectively.Conclusion The internal diameter of PVs of lower extremities in CVI patients has predictive value for its function.The treatment of the IPVs in the lower extremities may take some advantages to improve the clinical manifestations.
