腹腔镜辅助小切口TEP治疗成人腹股沟疝的临床分析

Analysis of curative effect of laparoscopic assisted small incision of TEP for inguinal hernia in adults

目的探讨腹腔镜辅助小切口腹膜外腹股沟疝修补术(TEP)术式治疗成人腹股沟疝的疗效及安全性。方法纳入2016年7月至2018年6月在本院接受治疗的122例腹股沟疝患者,随机分为观察组和对照组,每组61例。观察组行腹腔镜辅助小切口开放TEP术,对照组给予传统TEP术。比较两组患者的手术相关指标及术后并发症的发生情况。结果观察组手术时间短于对照组,差异有统计学意义(P<0.05)。两组的术中出血量、术后首次下床时间、住院时间、医疗总费用及疝复发率比较,差异均无统计学意义(P>0.05)。观察组患者的并发症发生率低于对照组(11.67%比27.87%),差异有统计学意义(P<0.05)。结论腹腔镜辅助小切口开放TEP可以取得和传统TEP相似的疗效,且有利于缩短手术时间和减少并发症,有一定临床应用价值。

Objective To analyze the clinical efficacy and safety of laparoscopic assisted small incision of total extraperitoneal hemiorrhaphy (TEP) for inguinal hernia in adults.Methods From July 2016 to June 2018,a total of 122 inguinal hernia patients in our hospital were randomly divided into observation group and control group,61 cases in each group.The observation group received laparoscopic assisted small incision of TEP,and the control group received TEP.The operation related indicators in the two groups were recorded and compared,and the occurrence of postoperative complications in the two groups were observed.Results The operation time in the observation group was shorter than that in the control group,with statistically significant difference (P<0.05).And the volume of intraoperative bleeding,first off-bed time after operation,hospital stay,total medical expense,and the rate of hernia recurrence in the two groups showed no statistically significant differences (P>0.05).The total incidence of complications in patients of the observation group was lower than that of the control group (11.67% vs.27.87%),with statistically significant difference (P<0.05).Conclusion Laparoscopic assisted small incision of TEP has the same efficacy as TEP,but laparoscopic assisted small incision of TEP has more advantages of shortening the operation time and reducing the incidence of complications,which has certain application value in clinic.
