共享决策在急诊冠心病患者中的应用观察

Application of sharing decision in emergency patients with coronary heart disease

目的探讨共享决策在急诊冠心病患者中的应用效果。方法选取本院急诊科2017年1月至2018年1月收治的158例冠心病患者作为观察组,选取2016年1月至2016年12月收治的155例冠心病患者作为对照组,对照组接受冠心病常规急诊干预,观察组接受急诊共享决策干预,比较两组患者的决策参与满意度评分、急救耗时、希望水平评价情况。结果观察组的决策参与满意度总分为(52.65±7.37)分,显著高于对照组的(50.84±7.88)分(P<0.05);观察组分诊评估时间(0.50±0.14)min、静脉采血时间(4.24±1.15)min、心电图检查时间(4.88±1.33)min、静脉通道建立时间(4.43±1.07)min显著短于对照组的分诊评估时间(3.42±0.78)min、静脉采血时间(8.39±2.17)min、心电图检查时间(6.23±1.75)min、静脉通道建立时间(7.12±2.11)min(P<0.05);观察组的Herth希望水平总分为(36.28±7.83)分,显著高于对照组的(33.87±8.44)分(P<0.05)。结论应用共享决策对急诊冠心病展开干预,对于改善患者决策参与满意度、缩短急救时间具有积极意义,同时有助于促进患者疾病希望水平的提高。

Objective To explore application effect of sharing decision in emergency patients with coronary heart disease(CHD).Methods A total of 158 CHD patients who were admitted to emergency department of our hospital from January 2017 to January 2018 were enrolled as observation group,155 CHD patients who were admitted from January 2016 to December 2016 were enrolled as control group.The control group was given routine emergency intervention for CHD,while the observation group was given emergency sharing decision intervention.The satisfaction score on decision participation,duration of first aid,and evaluation for hope level were compared between the two groups.Results The total satisfaction score on decision participation of the observation group was significantly higher than that of the control group[(52.65±7.37)points vs.(50.84±7.88)points](P<0.05).The triage assessment time,venous blood collection time,ECG examination time,and time of venous access establishment in the observation group were significantly shorter than those in the control group[(0.50±0.14)min vs.(3.42±0.78)min,(4.24±1.15)min vs.(8.39±2.17)min,(4.88±1.33)min vs.(6.23±1.75)min,(4.43±1.07)min vs.(7.12±2.11)min](P<0.05).The total score of Herth hope level of the observation group was significantly higher than that of the control group[(36.28±7.83)points vs.(33.87±8.44)points](P<0.05).Conclusion The application of sharing decision in intervention for emergency CHD is of positive significance for improving patients’satisfaction on decision participation,and shortening first aid time,while helps to improve their disease hope level.
