中药外用联合前列地尔用于下肢动脉闭塞症的疗效研究

Traditional Chinese medicine combined with alprostadil for lower extremity arterial occlusive disease

目的研究中药外用联合前列地尔治疗下肢动脉闭塞症的临床效果。方法选取本院100例下肢动脉闭塞症患者,随机数字表法分为对照组(n=50)和治疗组(n=50)。两组均行常规对症治疗,于此基础上对照组采用前列地尔治疗,治疗组采用中药外用联合前列地尔治疗。对比两组治疗效果、不良反应发生情况、治疗前后的症状体征[踝肱指数(ABI)、跛行距离、疼痛程度(VAS)]、生活质量(SF-36)。结果治疗结束后,两组患者的跛行距离、ABI均高于治疗前,且治疗组高于对照组;两组VAS评分均低于治疗前,且治疗组低于对照组(P<0.05);治疗组的治疗总有效率(94.00%)高于对照组(76.00%),χ^2=6.353,P<0.012;治疗结束后,两组SF-36评分均高于治疗前,且治疗组高于对照组(P<0.05);两组均无明显不良反应发生。结论中药外用联合前列地尔治疗下肢动脉闭塞症患者,能显著提高治疗效果,进一步改善患者的跛行距离,降低肢体缺血程度,减轻疼痛程度,有效提高患者生活质量,且安全性高。

Objective To study the clinical effect of external application of traditional Chinese medicine combined with alprostadil in the treatment of lower extremity arterial occlusive disease.Methods One hundred patients with lower extremity arterial occlusive disease were enrolled in our hospital.The random number table method was used to divide them into a control group(n=50)and a treatment group(n=50).Routine symptomatic treatment was performed in both groups.On the basis of this,the control group was treated with alprostadil,and the treatment group with topical Chinese medicine and alprostadil.The therapeutic effects,adverse reactions,preand post-treatment symptoms,and signs[ankle brachial index(ABI),limp distance,and pain level(VAS)],and quality of life(SF-36)were compared between the two groups.Results After the treatment,the limp distance,ABI,and SF-36 score were higher and the VAS was lower than those before the treatment in both groups,and were better in the treatment group than in the control group(P<0.05).The total effective rate was higher in the treatment group than in the control group(94.00%vs.76.00%,χ^2=6.353,P=0.012).No significant adverse reactions occurred in either group.Conclusion TCM combined with alprostadil in the treatment of lower extremity arterial occlusive disease can significantly improve the treatment effect and the patients'claudication distance and quality of life,and reduce the degree of limb ischemia and pain,and is safe.
