摘要
目的:探究连续θ短阵快速脉冲模式(cTBS)重复经颅磁刺激(rTMS)治疗伴幻听的精神分裂症疗效。方法:将124例伴有幻听的精神分裂症患者随机分成研究组(62例,完成61例)和对照组(62例,完成59例)。研究组进行4周的cTBS的rTMS治疗,对照组接受4周的伪刺激治疗。分别在干预前、干预2周和干预4周后对两组患者进行阳性和阴性症状量表(PANSS)、听幻觉评分量表(AHRS)、持续注意力测试(CPT)和Stroop色词测验(SCWT)评估。结果:①干预4周后研究组PANSS的阳性症状、阴性症状、一般精神病理和PANSS总分差值均明显高于对照组;②干预2周后和干预4周后,研究组的AHRS评分差值显著高于对照组;③干预4周后研究组SCWT中错报数、漏报数和反应时间与基线的差值均显著高于对照组;④干预4周后研究组的RTA、RTB、RTC及SIE与基线差值明显高于对照组。结论:cTBS模式的rTMS可改善精神分裂症的幻听症状及认知功能。
Objective:To explore the efficacy of continuous theta burst stimulation(cTBS)mode repetitive transcranial magnetic stimulation for schizophrenia with auditory hallucinations.Method:A total of 124 patients with schizophrenia accompanying auditory hallucinations were randomly divided into study group(62 cases,61 cases finished the study)and control group(62 cases,59 cases cases finished the study).The study group received a 4-week repetitive transcranial magnetic stimulation with CTBS mode,and the control group received 4 weeks of pseudo-stimulation.The two groups were evaluated using the Positive and Negative Symptoms Scale(PANSS),the Auditory Hallucinations Rting Scale(AHRS),the Continuous Performance Test(CPT)and the Stroop Color and Word Test(SCWT)before intervention,2 weeks after intervention and 4 weeks after intervention.Results:①After 4 weeks of intervention,the score reduction difference of positive,negative,general psychopathology and PANSS of the study group were significantly higher than those in the control group.②After 2,4 weeks of intervention,the score reduction difference in AHRS of the study group was significantly higher than that of the control group.③After 4 weeks of intervention,the difference in the number of false reports,the number of missed reports,and the reaction time from the baseline of the study group was significantly higher than those of the control group.④After 4 weeks of intervention,the difference of RTA,RTB,RTC,and SIE of the study group were significantly higher than those of the control group.Conclusion:The repetitive transcranial magnetic stimulation of cTBS mode can improve the auditory hallucinations and cognitive functions of schizophrenia.
