腮腺基底细胞腺瘤的CT表现特征

CT features of basal cell adenoma of the parotid gland

目的总结腮腺基底细胞腺瘤的CT特征,提高术前诊断水平。方法回顾性分析我院2010年3月~2018年7月经手术病理证实的35例腮腺基底细胞腺瘤患者临床资料及CT影像表现,并复习相关文献。结果基底细胞腺瘤以中老年女性好发,34例(97.14%)表现为无症状或腮腺区无痛性渐大肿块。33例(94.29%)单侧发病,单发病灶31例(88.57%);腮腺浅叶病灶27例(77.14%)。32例(91.43%)病灶界限清楚,3例边缘模糊;圆形/类圆形21例(60%)。基底节细胞腺瘤增强扫描,所有病灶动脉期迅速显著强化,动态增强呈“快进快出”模式8例,“快进慢出”模式22例(62.86%)。35例病灶中均匀性强化5例;不均匀强化30例(85.71%),其中坏死/囊变区14例(囊变区≥50%);4例病灶周缘见明显强化小结节样改变。结论腮腺基底细胞腺瘤的CT表现具有一定的特征性,结合患者临床表现有助于该病诊断与鉴别诊断。

Objective To summarize the CT features of basal cell adenoma(BCA)of parotid gland,in order to improve the level of preoperative diagnosis.Methods The clinical data and CT imaging findings of 35 cases of basal cell adenoma of parotid gland were retrospectively analyzed from March 2010 to July 2018 in our hospital,and related literatures were reviewed.Results BCA was more common in Mid-aged women,and 34 cases(97.14%)presented as asymptomatic or painless swelling in parotid area.There were 33 cases(94.29%)with unilateral onset and 31 cases with solitary lesions(88.57%).Superficial lobe lesions of parotid gland were found in 27 cases(77.14%).32(91.43%)had clear lesion boundaries and 3 had blurred margins.Circular/class circular in was found 21 cases(60%).The enhanced scan of BCA showed rapid and significant enhancement in all focal arterial phases,with dynamic enhancement showing"fast in and out"mode in 8 cases and"fast in and out"mode in 22 cases(62.86%).For 35 patients,5 cases displayed homogeneous enhancement,and 30 cases(85.71%)displayed heterogeneous enhancement,including 14 cases with necrotic/cystic region(the cystic region≥50%).In 4 cases,there was significant enhancement of nodule in the peripheral margin of lesions.Conclusion CT findings of BCA of parotid gland have certain characteristics,and combined with clinical manifestations is helpful for the diagnosis and differential diagnosis.
