摘要
目的探讨^18F-FDG PET/MR显像联合MR增强扫描在肝癌中的应用价值。方法选取2017年5月~2019年3月收治的肝癌患者55例,全部患者均接受^18F-FDG PET/MR显像,8例患者进行MR增强扫描,对影像学特点进行观察,并对病灶的最大标准化摄取值进行测量。结果在12例小肝癌患者、18例结节型肝癌和25例巨块型肝癌患者中,病灶延迟相SUVmax较早期升高的患者分别为6例、10例和23例,差异具有统计学意义(P<0.05);小肝癌早期相与延迟相SUVmax并不存在明显差异(P>0.05);结节型肝癌早期相与延迟相SUVmax无明显差异(t=1.528,P>0.05);巨块型肝癌早期相与延迟相SUVmax差异具有统计学意义(t=5.623,P<0.05);小肝癌、结节型肝癌和巨块型肝癌早期相SUVmax差异具有统计学意义(F=23.651,P<0.05);小肝癌、结节型肝癌和巨块型肝癌延迟相SUVmax差异具有统计学意义(F=27.489,P<0.05);小肝癌、结节型肝癌和巨块型肝癌延迟相SUVmax较早期相升高比例差异具有统计学意义(F=17.562,P<0.05)。8例患者通过MR增强扫描得到确诊。结论^18F-FDG PET/MR显像联合MR增强扫描在肝癌中的应用价值显著,可使肝癌得到有效的诊断,并使病情得到准确评估。
Objective To investigate the application value of ^18F-FDG PET/MR imaging combined with MR enhanced scanning in liver cancer.Methods A total of 55 patients with liver cancer admitted from May 2017 to March 2019 were selected.All patients underwent ^18F-FDG PET/MR imaging examination with MR enhanced scanning in 8 patients,we observed the imaging characteristics and measured the maximum standardized imaging value of the lesion.Results Among the 12 patients with small HCC,18 patients with nodular HCC and 25 patients with massive HCC,6 patients,10 patients and 23 patients with early-stage elevation of SUVmax in delayed phase of the lesion were detected,respectively,and the difference was statistically significant(P<0.05).There was no significant difference of SUVmax between early phase and delayed phase(P>0.05).There was no significant difference SUVmax of nodular HCC between early phase and delayed phase(t=1.528,P>0.05).The difference of SUVmax between early phase and delayed phase was statistically significant(t=5.623,P<0.05).The difference of SUVmax in the early phase between small liver cancer,nodular liver cancer and giant liver cancer was statistically significant(F=23.651,P<0.05).The difference of delayed phase SUVmax between small liver cancer,nodular liver cancer and giant liver cancer was statistically significant(F=27.489,P<0.05).The difference of the proportion of delayed phase SUVmax in small liver cancer,nodular liver cancer and massive liver cancer was statistically significant(F=17.562,P<0.05).Eight patients were confirmed by enhanced MR scanning.Conclusion The application of ^18F-FDG PET/MR imaging combined with MR enhanced scanning in hepatocellular carcinoma is of significant value,which can effectively diagnose hepatocellular carcinoma and accurately evaluate the disease.
