摘要
目的探讨在结合3D MEDIC WE和3D SPACE STIR序列提供显示神经根纤维束的对比研究方法。方法20例患者行3D MEDIC WE和3D SPACE STIR序列的腰丛神经根成像,最大密度投影(MIP),曲面重建(CPR)用以定性评价图像质量;多平面重组(MPR)用以充分显示神经根结构,定量测量两序列图像质量参数SNR,CNR以及CR。结果3D MEDIC WE和3D SPACE STIR两种序列均能不同程度清晰显示腰丛神经根。3D MEDIC WE对比3D SPACE STIR序列,影像评级占优(60 vs.55,P<0.05);3D SPACE STIR对比3D MEDIC WE序列,SNR(58.37±11.81 vs.10.71±0.80),CNR(28.05±7.85 vs.2.79±0.40),CR(6.75±0.63 vs.1.31±0.05)(P<0.05)占优。两序列联合诊断敏感性92.7%,特异性97.3%,准确率为92.7%。结论3D MEDIC WE和3D SPACE STIR序列应用于神经根成像效果显著,可不同程度不同重点直观清晰显示腰丛神经根走行及局部解剖细节,影像技术操作应考量图像可读性并结合图像技术参数为临床提供影像支持。
Objective In this paper,3D MEDIC WE and 3D SPACE STIR sequences are combined to provide a comparative research method for the display of nerve root fibrous bundles and to provide a reasonable titer ratio for the selection of MR sequences.Methods 20 patients with 3D MEDIC WE and 3D SPACE STIR sequence of lumbar plexus nerve root imaging,maximum density projection(MIP),curved surface reconstruction(CPR)were used to qualitative evaluation of image quality,multiple planar reconstruction(MPR)in order to fully display structure of nerve root,quantitative measurement of two sequence image quality parameter SNR,CNR and CR.Results Both 3D MEDIC WE and 3D SPACE STIR sequences can clearly show lumbar plexus nerve roots to varying degrees.Compared with 3D SPACE STIR sequence,3D MEDIC WE have a dominant image rating(60 vs.55,P<0.05).3D SPACE STIR compared 3D MEDIC WE sequence with SNR(58.37±11.81 vs.10.71±0.80),CNR(28.05±7.85 vs.2.79±0.40),CR(6.75±0.63 vs.1.31±0.05)(P<0.05).The combined diagnostic sensitivity of the two sequences is 92.7%,the specificity is 97.3%,and the accuracy is 92.7%.Conclusion 3D MEDIC WE and 3D SPACE WE STIR sequence are applied to the nerve root imaging effect significantly,which can be different levels of different key visual display lumbar plexus nerve root direction and local anatomical details of image and evaluating technical operation image readability and connecting with the image technology parameter for clinical imaging support.
