阿尔茨海默病和血管性痴呆认知功能与白质高信号及局部MRI弥散相关性分析 被引量:5

Correlation study on white matter hyperintensities,regional MRI diffusion,and cognitive function in Alzheimer’s disease and vascular dementia

摘要 目的通过比较阿尔茨海默病(AD)和血管性痴呆(VD)患者局部表现正常的脑白质(NAWM)MRI表观弥散系数(ADCs),研究脑白质高信号(WMH)、颈动脉粥样硬化和认知功能与ADCs的关系。方法23例AD患者和25例VD患者进行了脑MRI检查,以评估WMH体积和NAWM ADCs;同时进行颈动脉超声评估斑块严重程度和内膜中层厚度(intima-media thickness,IMT)及神经心理学成套测试。结果AD患者与VD患者在局部NAWM中的ADCs无明显差异;VD的WMH体积大于AD患者。胼胝体前部ADCs值与WMH体积呈正相关,颈动脉IMT越大,颞叶ADCs和WMH体积也越大。颞叶ADCs较高的患者记忆表现较差。结论研究数据显示,与WMH体积所表述的一样,ADCs较高的NAWM与微循环损害有关。此外,局部NAWM ADCs与记忆、结构性失用症和抽象推理领域等神经心理表现有不同的关联。
