气道支架置入术后感染的临床特点及影响因素分析

Clinical characteristics and influencing factors of infection after airway stent implantation

目的分析气道支架置入术后感染的临床特点及影响因素,为其诊疗提供临床依据。方法收集医院2015年1月至2016年12月104例置入气道支架患者的病历资料,采用描述性研究,对术后感染患者的临床特点进行描述;采用病例-对照研究方法和卡方检验对气道支架置入术后感染的影响因素进行回顾性分析。结果58例置入气道支架患者发生感染,占55.77%;症状以咳嗽、咳痰、气喘为主,病原菌中革兰阴性杆菌占70.59%;气道支架置入术后感染与患者的性别、年龄、原患疾病无关,与支架类型有关(P<0.05)。结论气道支架置入术后感染以咳嗽、咳痰、气喘为常见症状,少见发热。金属裸支架感染率较低,病原菌以革兰阴性菌为主。

Objective To analyze the clinical characteristics and influencing factors of infection after airway stent implantation so as to provide clinical basis for diagnosis and treatment of pulmonary infection.Methods The medical records of 104 patients with airway stent implanted in our hospital from January 2015 to December 2016 were collected.Descriptive study was used to describe the clinical characteristics of postoperative infection patients.The influence factors of infection after stent implantation were analyzed retrospectively by case-control study and chi-square test.Results 58 patients(55.77%)were infected with stent implantation.Patients with infection after airway stent implantation had cough,expectoration and asthma as the main symptoms,and gram-negative bacilli accounted for 70.59%of the pathogenic bacteria.Infection after stent implantation was not related to gender,age or original disease,but related to stent type(P<0.05).Conclusion Cough,expectoration and wheezing are the common symptoms of infection after stent implantation,and fever is rare.The infection rate of bare metal stent is low,and gram-negative bacteria are the main pathogens.
