摘要
目的比较120°儿童髋锁定加压钢板与空心钉内固定治疗儿童股骨颈骨折的临床疗效。方法回顾性分析2008年12月至2015年12月本院诊治的30例儿童股骨颈骨折患者资料,15例采用120°髋锁定加压钢板内固定(钢板组),15例采用空心钉内固定(空心钉组)。比较两组的手术时间、术中出血量、住院时间、术后颈干角丢失、骨折愈合时间,以及术后2年髋关节功能Sanders评分。结果钢板组获得平均28.3(24~32)个月随访,空心钉组术后获得平均28.4(24~33)个月随访。钢板组与空心钉组的手术时间、术中出血量、住院时间比较,差异无统计学意义(P>0.05);钢板组的骨折愈合时间、颈干角丢失均明显低于空心钉组,差异有统计学意义(P<0.05);随访24个月,钢板组髋关节功能恢复优于空心钉组(P<0.05)。结论相较于空心钉内固定术,儿童髋锁定加压钢板治疗儿童股骨颈骨折可取得满意疗效,具有骨折愈合时间短、颈干角丢失少等优势,术后髋关节功能恢复效果好。
Objective To compare the clinical efficacy of hip locking pressurized steel plate and hollow nail internal fixation in children with femoral neck fracture.Methods From December 2008 to December 2015,30 children with femoral neck fractures were treated,15 cases with 120°hip locking pressurized steel plate internal fixation(steel plate group)and 15 cases with hollow nail internal fixation(hollow nail group).The operation time,bleeding volume,hospitalization time,postoperative neck stem angle loss,fracture healing time,2 years hip function Sanders score of the two groups were compared.Results The steel plate group received an average of 28.3(24-32)months of follow-up,and the hollow nail group received an average of 28.4(24-33)months of follow-up after surgery.There was no significant difference in the time of operation,bleeding volume,and hospitalization time between the plate group and the hollow nail group(P>0.05).The time of fracture healing and the loss of dry neck angle of the plate group were significantly lower than that of the hollow nail group.The difference was statistically significant(P<0.05).After 24 months of follow-up,the hip joint recovery of plate group was better than that of hollow nail group(P<0.05).Conclusion Compared with hollow nail internal fixation,the children's hip locking pressurized grafting plate can achieve satisfactory results in the treatment of children's femoral neck fractures.It has the advantages of short healing time and less loss of neck dry angle,and the recovery of hip function is good after surgery.
