骨科-康复单元一体化康复治疗对单髁置换术后膝关节功能的影响

Efficacy of the integration of rehabilitation in orthopedic rehabilitation unit for surgical patients with unicompartmental knee arthroplasty

目的探讨单髁置换术后骨科-康复单元模式一体化康复治疗对膝关节功能的影响。方法将2014年3月至2018年2月本院单髁置换术治疗膝关节骨关节炎的患者32例,随机分为一体化康复组17例及对照组15例,分别进行骨科-康复单元模式一体化康复训练和传统模式康复训练。术后第1、3、6、12个月进行膝关节HSS评分及VAS评分。结果全部32例患者均得到1年以上随访,一体化康复组患者在术后第1、3、6个月时的HSS评分较对照组更高,两组比较差异均有统计学意义(P<0.05),术后第12个月时两组HSS评分差异无统计学意义(P>0.05);术后第1、3、6、12个月时VAS评分两组比较差异无统计学意义(P>0.05)。结论对单髁置换患者进行骨科-康复单元模式一体化康复治疗能较早促进膝关节功能恢复,提高步行能力,同时也不增加膝关节疼痛。

Objective To explore the effect of the integration of rehabilitation in orthopedic rehabilitation unit for the unicompartmental knee arthroplasty(UKA)patients.Methods A total of 32 patients with degenerative arthritis were treated with OxfordⅢUKA system between March 2014 and February 2018 in our hospital.They were randomized into the integration of rehabilitation group(17 cases)and the control group(15 cases).The integration of rehabilitation exercise program in orthopedic rehabilitation was adopted in the integration of rehabilitation group,while the conventional rehabilitation exercise was used in the control group.The assessment criteria for functional rehabilitation of knee included the 1,3,6,12 month postoperative hospital for special surgery knee score(HSS)and the visual analogue scales(VAS)at the corresponding period were also assessed.Results All the 32 patients were followed up for more than 1 year.Compared with the control group,the HSS scores of the integration of rehabilitation group were better in the 1,3,6 month postoperative follow-ups(P<0.05),while the difference of HSS scores in the 12 month between the integration of rehabilitation group and the control group were not significantly different(P>0.05).The VAS scores between the integration of rehabilitation group and the control group were not significantly different(P>0.05)in the 1,3,6,12 month postoperative follow-ups.Conclusion The integration of rehabilitation for the UKA patients promoted the recovery of knee joint function in orthopedic rehabilitation unit at early stage after operation,improving the capacity of walk with no more pain.
