经尿道前列腺电切术中低血压发生的影响因素分析与防护

Analysis and protection of influencing factors of hypotension occurrence during transurethral resection of prostate

目的探讨经尿道前列腺电切术中低血压的影响因素,总结预防性护理干预措施。方法选取2016年12月至2018年12月收治的148例择期前列腺电切术患者作为研究对象,其中14例(9.46%)术中发生低血压,其余134例未发生低血压。采用logistic回归模型确定影响术中低血压的因素。结果心血管病史、前列腺切除重量、术中低体温、失血量、手术时间、冲洗液温度、体位干预是经尿道前列腺电切术中发生低血压的相关因素;失血量、术中低体温、冲洗液温度、体位干预是经尿道前列腺电切术中发生低血压的独立影响因素。结论采取术中保暖,控制冲洗液速度和改良截石位等预防性护理措施,可减少术中低血压发生风险,保证手术过程安全性和手术治疗有效性。

Objective To explore the influencing factors of hypotension in transurethral resection of prostate,and to summarize preventive nursing interventions.Methods We selected 148 cases of patients undergoing elective prostatectomy from December 2016 to December 2018 as a study object.14 cases(9.46%)of which had hypotension during surgery and 134 cases had no hypotension.Logistic regression models were used to determine the influencing factors of intraoperative hypotension.Results History of cardiovascular,resection weight of prostate,intraoperative hypothermia,blood loss,operative time,temperature of irrigating fluid,and postural intervention were the related factors associated with hypotension during transurethral resection of the prostate.Blood loss,intraoperative hypothermia,temperature of irrigating fluid,and postural intervention were independent influencing factors associated with hypotension during transurethral resection of the prostate.Conclusion Preventive nursing measures such as intraoperative warmth,controlled the speed of irrigating fluid and improved lithotomy position could reduce the risk of intraoperative hypotension and ensure the safety of the surgical procedure and the effectiveness of surgical treatment.
