腹腔镜胆囊切除术治疗胆囊颈部结石嵌顿合并急性化脓性胆囊炎患者的临床效果

Clinical effect of laparoscopic cholecystectomy in the treatment of patients with calculus incarcerated in the neck of gallbladder and acute suppurative cholecystitis

目的探究腹腔镜胆囊切除术治疗胆囊颈部结石嵌顿合并急性化脓性胆囊炎患者的临床效果。方法将我院收治的72例胆囊颈部结石嵌顿合并急性化脓性胆囊炎患者根据治疗方案不同分为对照组与观察组,各36例。对照组给予开腹胆囊切除术治疗,观察组给予腹腔镜胆囊切除术治疗。对比两组的治疗效果。结果观察组的出血量、手术时间、住院时间、排气时间及下床活动时间均优于对照组(P<0.05)。术后1 d,两组AST、ALT水平均升高,但观察组低于对照组(P<0.05)。观察组并发症总发生率低于对照组(P<0.05)。结论腹腔镜胆囊切除术治疗胆囊颈部结石嵌顿合并急性化脓性胆囊炎具有创伤小、出血少、术后恢复快、安全性高等优势,并可减轻肝功能损伤。

Objective To explore the clinical effect of laparoscopic cholecystectomy in the treatment of patients with calculus incarceratied in the neck of gallbladder and acute suppurative cholecystitis.Methods A total of 72 patients with calculus incarceratied in the neck of gallbladder and acute suppurative cholecystitis admitted in our hospital were divided into control group and observation group according to the different treatment plans,with 36 cases in each group.The control group was treated with open cholecystectomy,while the observation group was treated with laparoscopic cholecystectomy.The treatment effects were compared between the two groups.Results The bleeding volume,operation time,hospitalization time,exhaust time and time of getting out of bed in the observation group were better than those in the control group(P<0.05).At one day after operation,the levels of AST and ALT increased in both groups,but those in the observation group were lower than the control group(P<0.05).The total incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion Laparoscopic cholecystectomy for calculus incarceratied in the neck of gallbladder and acute suppurative cholecystitis has the advantages of less trauma,less bleeding,quicker recovery after operation,higher safety,and can alleviate liver function damage.
