以功能训练为重点的循证护理对行全膝关节置换术患者肢体功能及精神状态的影响

Effect of evidence-based nursing with functional training as focus on limb function and mental state in patients undergoing total knee arthroplasty

目的观察以功能训练为重点的循证护理对行全膝关节置换术患者肢体功能及精神状态的影响。方法选取在我院进行全膝关节置换术的112例患者为研究对象,按护理方式不同将其分为对照组(56例,常规护理)和观察组(56例,以功能训练为主的循证护理)。比较两组关节功能的恢复情况及精神状态。结果观察组膝关节恢复优良率高于对照组(P<0.05)。术后3、5、7 d,两组患者的膝关节周径、膝关节屈曲度及关节肿胀程度均改善,且观察组优于对照组(P<0.05)。术后7 d及术后1、3、6个月,两组患者的HSS评分、步频、FIM评分均升高,且观察组高于对照组(P<0.05)。术后3、5、7 d,两组患者的NRS评分均降低,且观察组低于对照组(P<0.05)。结论给予膝关节置换术患者以功能训练为重点的循证护理,能显著促进膝关节功能的恢复,减轻疼痛程度,利于术后恢复。

Objective To observe the effect of evidence-based nursing with functional training as focus on limb function and mental state in patients undergoing total knee arthroplasty.Methods One hundred and twelve patients who underwent total knee arthroplasty in our hospital were selected as the research objects and divided into control group(56 cases,routine nursing)and observation group(56 cases,evidence-based nursing with functional training as focus)according to different nursing methods.The recovery of joint function and mental state were compared between the two groups.Results The excellent and good rate of knee joint recovery in the observation group was higher than that in the control group(P<0.05).On the 3rd,5th and 7th day after operation,knee circumference,knee flexion and joint swelling improved in both groups,and those in the observation group were better than the control group(P<0.05).The score of HSS,stride rate and FIM score in the two groups increased at 7 days and 1,3 and 6 months after operation,and those in the observation group were higher than the control group(P<0.05).At 3,5 and 7 days after operation,the scores of NRS in both groups decreased,and those in the observation group were lower than the control group(P<0.05).Conclusion Evidence-based nursing with functional training as focus applied to patients undergoing total knee arthroplasty can significantly promote the recovery of knee joint function,reduce the degree of pain,and facilitate the recovery after operation.
