Analysis and exploration of clinical use of the invasive arterial pressure sensor system in monitoring intracranial pressure changes through external ventricular drainage tubes

摘要 目的:探讨分析临床上运用有创动脉压力传感器系统通过外接脑室外引流管监测颅内压力的变化。方法:选取2015年3月至2016年4月收治的60例潜在颅内压增高的患者随机分为观察组和对照组。对照组给予常规生命体征检测,观察组给予有创动脉压力传感器系统通过外接脑室外引流管监测颅内压力变化,比较两组临床预后、临床指标及并发症发生情况。结果:观察组临床预后良好率为80.00%,较对照组显著提高(P<0.05);观察组与对照组相比,甘露醇使用时间显著缩短、用量显著减少,开颅例数显著降低,入院至开颅时间显著缩短(P<0.05);观察组肾功能损伤、肺部感染、电解质紊乱、颅内感染、应激性溃疡发生率分别为3.33%、0.00%、10.00%、16.67%、6.67%,较对照组显著降低(P<0.05)。结论:对于潜在颅内压增高的患者给予有创动脉压力传感器系统通过外接脑室外引流管监测颅内压力可显著改善临床预后,合理指导脱水剂使用,降低开颅比例和相关并发症的发生,且成本低,减轻患者的经济负担,尤其适合在基层医院开展。 Objective:To explore the clinical application of the invasive arterial pressure sensor system in monitoring intracranial pressure changes through external ventricle drainage tubes. Methods:60 patients with potential intracranial hypertension admitted from March 2015 to April 2016 were chosen and randomly divided into the observation group and the control group. The control group was monitored by routine vital signs, while the observation group was monitored by the invasive arterial pressure sensor system to detect intracranial pressure changes through external ventricular drainage tubes, with the clinical prognosis, clinical indicators and complications compared. Results:The good clinical prognosis rate of the observation group was 80.00 %, significantly higher than that of the control group(P<0.05). Compared with the control group, the mannitol use time was significantly shortened, the dosage was significantly reduced, and the number of craniotomy was significantly reduced. The time from hospital admission to craniotomy was significantly shortened(P<0.05). The incidence of renal function damage, pulmonary infection, electrolyte imbalance, intracranial infection and stress ulcer in the observation group was 3.33 %, 0.00 %, 10.00 %, 16.67 % and 6.67 % respectively, which was significantly lower than that in the control group(P<0.05);Conclusion:The intra-arterial pressure sensor system can be used to monitor clinical intracranial pressure in patients with potential intracranial hypertension through external ventricular drainage tubes, improving clinical prognosis, rationally guiding dehydrating agent, reducing the incidence of craniotomy, related complications, cost and the economic burden of patients, which is especially suitable for primary hospitals.
