摘要
目的了解慢性阻塞性疾病急性加重期(AECOPD)男性患者的营养状况,并分析其与炎性反应及肺功能之间的相关性。方法通过对浙江衢州市柯城区人民医院2016年1月-2017年12月期间365例男性AECOPD患者相关指标回顾性调查,分析其营养状况、炎性反应及肺功能之间的相关性。结果从血红蛋白、白蛋白、前白蛋白及BMI结果看,AECOPD男性患者营养不良的比例分别为13.7%、29.6%、80.3%、30.4%。NRS2002结果显示,25.5%的患者存在营养风险。无营养风险组患者体重、BMI、白蛋白、前白蛋白、血红蛋白、FVC%Pred、FEV1%Pred及FEV1/FVC显著高于有营养风险组(P<0.05),C反应蛋白显著低于有营养风险组(P<0.05)。Pearson相关分析显示,C反应蛋白水平与血红蛋白、白蛋白、前白蛋白、BMI水平均呈负相关(P<0.05)。BMI、白蛋白、前白蛋白与FVC%Pred、FEV1%Pred、FEV1/FVC呈正相关。C反应蛋白、白细胞数与FVC%Pred、FEV1%Pred、FEV1/FVC呈负相关(P<0.05)。结论AECOPD男性患者肺功能与营养状况、炎性水平密切相关。另外,营养状况对患者炎性水平有重要影响。
Objective To investigate the nutritional status of AECOPD male patients,and analyze its relation with inflammation and pulmonary function.Methods The clinical data of 365 AECOPD male patients,from January 2016 to December 2017,were retrospectively reviewed to analyze the correlation of nutriture,inflammation and pulmonary function.Results The prevalence of malnutrition classified by hemoglobin,serum albumin,prealbumin,and BMI were 13.7%,29.6%,80.3%,30.4%respectively.Nutritional risk screening 2002 Results suggested that 25.5%AECOPD male patients have nutritional risk.The value of body weight,BMI,serum albumin,prealbumin,hemoglobin,FVC%Pred,FEV1%Pred and FEV1/FVC in nutritional risk group were significantly lower than those in no nutritional risk group,C-reactive protein was significantly higher compared with no nutritional risk group.The Pearson correlation analysis showed that the C-reactive protein was negatively related with BMI,hemoglobin,serum albumin and prealbumin.BMI,serum albumin and prealbumin were positively related with FVC%Pred,FEV1%Pred and FEV1/FVC.The inflammatory biomarkers,including C-reactive protein and WBC,were negatively related with FVC%Pred,FEV1%Pred and FEV1/FVC.Conclusions The pulmonary function of AECOPD male patients was strongly correlated with nutritional status,inflammation.Additionally,the nutritional status have important effects on inflammatory responses.
