2μm激光前列腺剜除术治疗复发性前列腺增生的临床疗效探讨

Clinical efficacy of Prostate Enucleation with Two-microns Laser in Treating Recurrent Benign Prostatic Hyperplasia

目的探讨2μm激光前列腺剜除术治疗复发性前列腺增生的临床疗效。方法选择潢川县人民医院自2017年1月-2017年12月收治的53例复发性前列腺增生患者作为研究对象,患者均行2μm激光前列腺剜除术治疗,对比患者治疗前后的国际前列腺症状评分、最大尿流量、残余尿量,并统计患者手术时间、住院时间、术后并发症发生率及术后1年的复发率。结果治疗后,患者国际前列腺症状评分(4.12±0.56)分、残余尿量(20.62±4.27)mL均显著低于治疗前(29.34±2.87)分、(146.51±6.8)mL,治疗后,患者生活质量评分(86.74±8.25)分、最大尿流量(17.62±2.45)mL/s均显著高于治疗前(41.35±5.92)分、(7.41±3.16)mL/s,前后对比具有统计学意义(P<0.05);患者平均手术时间为(40.2±7.0)min、平均住院时间为(6.4±0.7)d,术后尿管拔除后,1例患者出现尿频、尿急反应,术后2周,1例患者发生短暂尿失禁反应,术后并发症发生率为3.77%,术后对患者进行1年随访,未有患者再复发。结论对复发性前列腺增生患者采用2μm激光前列腺剜除术治疗,不仅能有效改善患者前列腺增生症状和降低其病症再次复发率,且患者术后并发症发生率较低。

Objective To investigate the clinical effect of 2 micron laser enucleation of prostate in the treatment of recurrent prostatic hyperplasia.Methods 53 patients with recurrent prostatic hyperplasia admitted to our hospital from January 2017 to December 2017 were selected as the study subjects.All patients underwent 2-micron laser prostatectomy.Compared the score of international prostate symptom,maximum urine flow and residual urine before and after treatment,and calculated the operative time,length of hospital stay,incidence of postoperative complications and recurrence rate 1 year after the operation.Results After treatment,the international prostate symptom score(4.12±0.56)and residual urine volume(20.62±4.27)mL were significantly lower than before treatment(29.34±2.87),(146.51±6.8)mL,and the quality of life score(86.74±8.25)and the maximum urine flow(17.62±2.45)mL/s were significantly higher than before treatment(41.35±5.92)and(7.41±3.16)mL/s,the comparison was statistically significant(P<0.05).The average operation time of the patients was(40.2±7.0)min and hospitalizing time was(6.4±0.7)d.After the catheter removal,1 patient had frequent and urgent urination reactions;2 weeks after the surgery,1 patient had transient urinary incontinence reactions,and the incidence of postoperative complications was 3.77%;followed up for 1 year after the operation,and no patient had recurrence.Conclusions Prostate enucleation with two-microns laser in treating recurrent benign prostatic hyperplasia can not only effectively improve the symptoms and reduce the recurrence rate of BPH,but also lower the incidence of complications.
