摘要
目的检测急性胰腺炎(AP)患者血清自噬相关蛋白Beclin1、白介素-18(IL-18)的表达并探讨其与AP病情严重程度的相关性。方法全自动生化检测仪测定血清淀粉酶(Amy)含量,采用酶联免疫吸附试验法(ELISA)检测血清Beclin1、IL-18含量,并用改良CT严重指数(MCTSI)、急性生理与慢性健康评分Ⅱ(APACHE-Ⅱ)评分方法进行评分。结果重症急性胰腺炎(SAP)组及轻型急性胰腺炎(MAP)组患者血清Amy、Beclin1及IL-18水平较对照(Ctr)组升高(P<0.01)。SAP组血清Amy、Beclin1及IL-18水平较MAP组升高(P<0.01);Ctr组MCTSI、APACHE-Ⅱ评分基本为0,AP发病时MCTSI、APACHE-Ⅱ评分增高,MAP和SAP与Ctr比较,MCTSI、APACHE-Ⅱ评分增高;并且SAP患者MCTSI、APACHE-Ⅱ评分较MAP患者MCTSI、APACHE-Ⅱ评分升高(P<0.01)。血清Beclin1与血清IL-18水平及MCTSI和APACHE-Ⅱ评分均相关,相关系数r分别为0.918 9、0.977 1和0.926 0。结论在AP患者中,血清Beclin1、IL-18水平及MCTSI、APACHE-Ⅱ评分升高,血清Beclin1与血清IL-18水平及MCTSI、APACHE-Ⅱ评分均相关,血清Beclin1与AP的发生及其程度密切相关,在评估及预测AP严重程度方面有一定的作用。
Objective To detect the serum autophagy-associated protein Beclin1, interleukin-18(IL-18) among the acute pancreatitis(AP) patient in the clinic and discuss its relevance with the severe extent of AP. Methods The Amy content was detected by full-automatic biochemical detector, the contents of serum Beclin1 and IL-18 detected by enzyme linked immunosorbent assay(ELISA), and the evaluation was done by modified CT severity index(MCTSI) and acute physiology and chronic health evaluation-Ⅱ(APACHE-Ⅱ) scoring methods. Results The level of serum Amy, Beclin1 and IL-18 among patients in severe acute pancreatitis(SAP) group and mild acute pancreatitis(MAP) group increased compared with the control(Ctr) group(P<0.01). The level of serum Amy, Beclin1 and IL-18 in the SAP group increased compared with MAP group(P<0.01);the scores of MCTSI and APACHE-Ⅱ in the Ctr group were basically zero;the scores of MCTSI and APACHE-Ⅱ increased during the paroxysm of AP, the MAP and SAP group had higher marks in the scores of MCTSI and APACHE-Ⅱ compared with Ctr;and scores of MCTSI and APACHE-Ⅱ in SAP patients were higher than that of MAP patients(P<0.01). The level of serum Beclin1 and IL-18 were correlated with both MCTSI and APACHE-Ⅱ score, the coefficient of association r was 0.918 9, 0.977 1 and 0.926 0, respectively. Conclusion Among the clinical AP patients, the level of serum Beclin1 and IL-18, the increased score of MCTSI and APACHE-Ⅱ, the level of serum Beclin1 and IL-18, as well as the score of MCTSI and APACHE-Ⅱ are all correlated. The serum Beclin1 is closely correlated with the paroxysm and extent of AP, hence it plays a certain role in the evaluation and prediction of predicting the severe extent of AP.
