钱先教授治疗强直性脊柱炎经验浅析

Professor QIAN Xian’s Experience in Treating Ankylosing Spondylitis

强直性脊柱炎属“大偻”“骨痹”等范畴,钱先教授认为本病病位在肾与脊柱,和肝、脾胃密切相关,属本虚标实之证,本虚为肾虚督空,标实为风、寒、湿、热、痰、瘀痹阻经络。治疗应分期而治,疾病初期应注重祛风除湿,散寒通络,疾病中期根据兼证的不同,选用不同治法;疾病后期注重化痰袪瘀,但在疾病的各个时期都主张补肾强督为治疗基本原则,并重视炎性肠病和葡萄膜炎等兼夹证的治疗。

Ankylosing spondylitis(AS)belongs to the categories of“Dalou”and“Gubi”in TCM.Professor Qian regards the disease is located in the Kidney and spine,and is closely related to the Liver,the Spleen and the Stomach,the feature of the disease is deficiency in nature and excess in superficiality.The deficiency is due to the Kidney deficiency and deficiency of the Governor Vessel.Pathogenic wind,cold,dampness,heat,phlegm,blood stasis are the excess in superficiality.The treatment should be carried out by stages.In the early stage,dispelling wind and eliminating dampness,as well as dispelling cold and dredging collaterals should be paid attention to.In the middle stage,different treatment methods are selected according to different concurrent syndromes.At the later stage,dissolving phlegm and removing blood stasis is attached importance to.However,in every period of the disease,the basic principle is to tonify the Kidney and strengthen the Governor Vessel,at the same time,the treatments of inflammatory bowel disease and uveitis are paid attention to.
