miR-195检测在先兆子痫患者中的临床意义

摘要 目的探讨先兆子痫(PE)患者孕早期和孕中期血清miR-195水平检测的临床意义。方法选取收治的PE患者98例为研究组,同期产检的健康孕妇60例为正常妊娠组,分别在孕早期(10~14周)、孕中期(20~24周)收集血液样本,采用实时荧光定量PCR方法检测血清miR-195表达水平,分析PE患者孕早期和孕中期血清miR-195水平检测的临床意义。结果 PE孕早期组、PE孕中期组血清miR-195水平显著低于同孕期正常妊娠组,且PE孕中期组患者血清miR-195水平明显低于PE孕早期组(P<0.05);PE孕早期组、PE孕中期组患者SBP、DBP、TC、TG、LDL-C均明显高于同孕期正常妊娠组(P<0.05),且PE孕中期组患者SBP、DBP、TG、LDL-C均显著高于PE孕早期组(P<0.05);Pearson分析显示,PE孕早期、孕中期患者miR-195表达水平与DBP、TG呈正相关(r值分别为0.382、0.452、0.337、0.431,P<0.05),孕中期miR-195与LDL-C呈负相关(r=-0.537,P<0.05),与SBP、TC、HDL-C无相关性(P>0.05);绘制ROC曲线显示,miR-195在PE孕早期的AUC为0.797,诊断敏感性为67.39%,特异性为86.67%(P<0.05);miR-195在PE孕中期的AUC为0.932,诊断敏感性为80.77%,特异性为91.69%(P<0.05)。结论 PE孕早、中期患者普遍存在血清miR-195水平降低现象,并随着妊娠时间延长miR-195水平降低加剧,血清miR-195在孕中期的敏感性及特异性较高,可能是PE预测的潜在生物标记物。 Objective To investigate the clinical significance of serum levels of miR195 in patients with preeclampsia(PE) during the first trimester and second trimester. Methods Ninety-eight patients with PE who were treated in our hospital were enrolled as research group and sixty healthy pregnant women, who underwent prenatal examination during the same period, were enrolled as normal pregnancy group. The blood samples were collected at first trimester(10~14 weeks) and the second trimester(20~24 week), respectively. PCR was used to detect the expression of serum level of miR195. And the clinical significance of the serum level of miR195 at first trimester and the second trimester was analyzed.Results The serum levels of miR195 in PE first trimester group and PE second trimester group were significantly lower than that in normal pregnancy group, and the serum level of miR195 in PE second trimester group was significantly lower than that in PE first trimester group(P<0.05). Moreover, the levels of SBP, DBP, TC, TG and LDLC in PE first trimester group and PE second trimester group were significantly higher than that in normal pregnancy group, and the levels of SBP, DBP, TC, TG and LDLC in PE second trimester group was significantly higher than that in PE first trimester group(P<0.05). Furthermore, Pearson analysis showed that the expression levels of miR195 in PE first trimester group and PE second trimester group were positively correlated with DBP and TG(r=0.382,0.452, 0.337,0.431,respectively,P<0.05),which were negatively correlated with LDLC(r=-0.537,P<0.05), however,which were not correlated with SBP, TC and HDLC was found(P>0.05).In addition, the AUC, sensitivity, specificity of miR195 in PE first trimester group were 0.797, 67.39% and 86.67%, respectively(P<0.05). And the AUC, sensitivity, specificity of miR195 in PE second trimester group were 0.932, 80.77% and 91.69%, respectively(P<0.05).Conclusion The decrease of serum levels of miR195 is common in PE patients during the first trimester and second trimester, which aggravates with the prolongation of pregnancy time. The sensitivity and specificity of serum miR195 in the second trimester is relatively higher, which maybe a potential biomarker of PE prediction.
