β-HCG指导异位妊娠治疗方式的临床研究

The significance of Beta-HCG value in guiding treatment way of ectopic pregnancy

目的评价β-人绒毛膜促性腺激素(β-HCG)水平用于指导异位妊娠治疗方式的临床研究价值。方法回顾性分析2016年4月至2019年1月收治的350例异位妊娠患者,按照治疗方法不同分为2组,甲组200例实施药物保守治疗,乙组150例实施手术治疗,比较不同血β-HCG水平下不同方法的治疗效果。结果乙组治疗3、7、14 d血清β-HCG值下降低于甲组(P<0.05),甲组治疗后不同水平β-CHCG值恢复正常时间多于乙组(P<0.05),治疗6个月后2组低、中水平的妊娠率差异有统计学意义(P<0.05)。血β-CHCG同水平下手术治疗效果高于药物保守治疗(P<0.05)。结论血β-CHCG值低则保守治疗,手术效果较好,对于依存性好、处于相对禁忌的特殊患者,可以适当放宽适应证,注重个体化方案,也取得很好的效果;临床可通过血β-CHCG值指导临床选择合理的异位妊娠的治疗方法。

Objective To investigate the significance of β-HCG value in guiding treatment way of ectopic pregnancy.Methods The clinical data about 350 patients with ectopic pregnancy who were admitted and treated in our hospital from April 2016 to January 2019 were retrospectively analyzed.These patients were divided into two groups according to different treatment methods.The 2,000 patients in group A received drug conservative treatment,and the other 150 patients in group B received surgical treatment.The therapeutic effects at different levels of β-HCG were observed and compared between the two groups.Results As compared with those in group A,the serum levels of β-HCG at 3 d,7 d and 14 d after treatment in group B were significantly decreased(P<0.05),and the time to return to normal levels of β-HCG after treatment in group A was longer than that of group B(P<0.05).At 6 months after treatment,there were significant differences in pregnancy rates at middle and low levels of β-HCG between the two groups(P<0.04).Moreover the therapeutic effects in the same levels of β-HCG in group B were superior to those in group A(P<0.05).Conclusion The drug conservative treatment should be used in ectopic pregnancy patients with low levels of β-HCG,and the serum levels of β-HCG can be used to guide the clinical selection of reasonable treatment way for ectopic pregnancy.
