本地区86家医疗机构输血前检测盲样比较

Comparison result of blind sample detection before blood transfusion in 86 local medical institutions

目的了解本地区临床用血医疗机构ABO、RhD血型鉴定以及输血前感染四项检测结果的正确性和规范性。方法对本地区86家市属临床用血医疗机构均发放相同的5份和2份血液样本分别进行ABO、RhD血型和输血前感染四项的检测,并对上报的血型结果及输血前感染四项结果进行分析。结果 ABO血型正定型结果正确率为63.9%~100%,凝集强度正确率为63.9%~98.8%;ABO血型反定型结果正确率为84.9%~98.8%,凝集强度正确率为84.6%~97.6%;RhD血型鉴定结果和凝集强度正确率均为100%。不同的方法学检测对输血前感染四项阳性和阴性样本检测结果一致性较好,但对弱阳性样本检测结果差别较大。结论实验室间比对可以客观地评价各实验室的实验结果,发现检测中存在的一些问题并加以改进,使各实验室间的结果具有可比性,是室间质量评价的一种控制检验结果准确性的补充性方法,可促进实验室持续改进检测质量。

Objective To investigate the correctness and standardization of ABO and RhD blood typing as well as four tests for infection before blood transfusion in local medical institutions. Methods The same 5 and 2 blood samples were distributed to 86 clinical blood medical institutions in the region for ABO, RhD blood typing pretransfusion infection indexes test, respectively.The results were statistically analyzed.Results The correct rate of ABO direct typing and agglutination intensity were 63.9%~100% and 63.9%~98.8%, respectively.And the correct rate of ABO reverse typing and agglutination intensity were 84.9%~98.8% and 84.6%~97.6%, respectively.The correct rate of RhD blood typing and agglutination intensity were 100%.In addition, different methodological tests showed good agreement on the results of positive and negative sample test, but the results of weak positive samples were different.Conclusion Interlaboratory comparisons can objectively evaluate the experimental results of each laboratory, find problems in testing and improve them, which is a complementary method of controlling the accuracy of test results for external quality assessment.
