快速康复外科理念个体化护理在腹腔镜辅助全胃切除术患者中的效果

Effects of individualized nursing based on the concept of fast rehabilitation surgery in patients undergoing laparoscopic assisted total gastrectomy

目的分析快速康复外科理念个体化护理在腹腔镜辅助全胃切除术患者中的效果。方法选择2017年1月至2018年1月收治的需行腹腔镜辅助全胃切除术患者84例,随机分为对照组和研究组,每组42例。对照组实施常规护理,研究组实施快速康复外科理念的个体化护理。比较2组患者术后恢复情况、术后舒适状况、住院时间、治疗费用及并发症发生情况。结果研究组患者术后肠道恢复通气时间、进食时间、首次排便时间、首次下床时间、住院时间较对照组提前,住院时间和治疗费用明显低于对照组,差异统计学意义(P<0.05);研究组术后GCQ评分显著高于对照组,差异有统计学意义(P<0.05);研究组患者并发症发生率为4.76%显著低于对照组的19.05%,差异有统计学意义(P<0.05)。结论对行腹腔镜辅助全胃切除术患者实施快速康复外科理念的个体化护理效果理想,有利于患者术后肠道功能的快速恢复,缩短住院时间,减少治疗费用及降低并发症发生率,提高患者舒适度,有利于患者的康复以及提升护理质量。

Objective To analyze the effects of individualized care based on the concept of fast rehabilitation surgery in patients undergoing laparoscopic assisted total gastrectomy.Methods Eighty-four patients who underwent laparoscopic-assisted total gastrectomy in our hospital from January 2017 to January 2018 were randomly divided into the control group and the research group, with 42 patients in each group. The patients in the control group were treated with conventional nursing and the patients in the research group were given individualized care based on the concept of fast track surgery.The postoperative recovery, postoperative comfort, length of hospitalization,treatment expenses,and complications were observed and compared between the two groups.Results After surgery the intestinal ventilation time,feeding time,first defecation time,first time to get out of bed,and length of hospitalization after surgery in research group were significantly earlier or shorter than those in control group,and the treatment expenses were significantly lower than those in control group(P<0.05).The GCQ scores in research group were significantly higher than those in control group(P<0.05).The incidence rate of complication in research group was 4.76%,which was significantly lower than that(19.05%) in control group(P<0.05).Conclusion The implementation of individualized nursing based on the concept of fast track surgery is favorable in patients undergoing laparoscopic assisted total gastrectomy.It is beneficial to the rapid recovery of postoperative intestinal function,which can also shorten duration of hospitalization,reduce treatment expenses and the incidence of complications,and improve the comfort of the patients.Therefore,it is conducive to the rehabilitation of patients and improvement of the quality of care.
