上海市高行社区老年人群糖尿病影响因素调查分析

Prevalence and risk factors of diabetes mellitus among elderly people in Gaohang community of Shanghai

目的调查目标人群糖尿病的患病率现状并探讨糖尿病高发的相关危险因素。方法采用整群随机抽样的方法来选取研究对象并进行回顾性调查分析,应用简单线性回归分析各个危险因素之间的相关性,采用Logistic多元回归模型来探讨导致糖尿病高发的相关危险因素。结果共有4 650人参与了本研究,其中男性2 090人,女性2 560人,糖尿病组中BMI、收缩压、Ca 2+、WBC等指标均高于对照组(P<0.05);线性分析提示:HbA1c水平与BMI、SBP、Ca 2+、UN、WBC均呈弱相关(P<0.05);与HDL-C呈负相关;Logistic回归模型提示:较高的BMI、高血压、较高的血清Ca 2+、较高的血尿素氮、较低的高密度脂蛋白胆固醇都是糖尿病的危险因素,其对应的OR值与95%CI分别为1.096(1.074~1.119)、1.006(1.003~1.010)、8.307(3.857~17.892)、1.068(1.024~1.114)和0.533(0.439~0.647)。结论上海市高行社区老年居民糖尿病的患病率为35.2%,较高的白细胞水平、血清Ca 2+含量、肥胖可能是导致糖尿病高发的危险因素。

Objective To survay the prevalence and risk factors of diabetes among elderly people in Shanghai Gaohang community.Methods A cluster random sampling was used to select subjects from Gaohang community of Shanghai Pudong New Area and a survey on the prevalence of diabetes was conducted.Logistic regression model was used to explore the risk factors associated with high incidence of diabetes.Results Totally of 4 650 people participated in the study,including 2 090 males and 2 560 females.The BMI,systolic blood pressure,Ca 2+,WBC in the diabetic group were significantly higher than those in the control group(P<0.05).The linear analysis indicated that the HbA1c level had a positive correlation with BMI,SBP,Ca 2+,UN and WBC(P<0.05);and had a negative correlation with HDL-C.The Logistic regression model suggested that higher body mass index(BMI),hypertension,higher serum calcium ion,higher blood urea nitrogen and lower high-density lipoprotein cholesterol were independent risk factor of diabetes,and the corresponding OR values and 95%CI were 1.096(1.074-1.119),1.006(1.003-1.010),8.307(3.857-17.892),1.068(1.024-1.114)and 0.533(0.439-0.647),respectively.Conclusion The prevalence of diabetes among elderly residents in Gaohang community in Shanghai is 35.2%,and high levels of white blood cells,serum calcium ion content and obesity may be risk factors for diabetes.
