腔镜手术微创治疗老年肺癌的疗效及术后肺功能影响

摘要 目的分析老年肺癌患者采用腔镜手术治疗的临床疗效及对患者术后肺功能产生的影响。方法方便选取2014年1月—2019年1月该院收治的63例老年肺癌患者为研究对象,随机分为两组,对照组采用传统开胸肺癌切除术治疗,观察组采用腔镜手术治疗,对比两组手术前后肺功能、围术期指标以及并发症发生情况。结果观察组治疗后FEVl/FVC为(79.52±6.85)%,高于对照组的(70.12±6.23)%,FVC为(2.75±0.59)L,高于对照组的(2.45±0.35)L,FEVl为(2.99±0.89)L,高于对照组的(2.71±0.26)L,差异有统计学意义(t=8.457、9.637、10.637,P<0.05);与对照组对比,观察组住院时间、胸腔引流时间更短,术中出血量更少,差异有统计学意义(t=5.669、9.669、12.529,P<0.05);观察组并发症发生率为6.25%,低于对照组的32.26%,差异有统计学意义(χ^2=6.907,P<0.05)。结论老年肺癌患者采用腔镜手术治疗的临床疗效显著,利于改善患者术后肺功能。 Objective To analyze the clinical efficacy of laparoscopic surgery in elderly patients with lung cancer and its effect on postoperative pulmonary function.Methods Sixty-three elderly patients with lung cancer admitted to the hospital from January 2014 to January 2019 were convenient selected and enrolled in the study.They were randomly divided into two groups.The control group was treated with conventional open-thoracic lung cancer resection.The observation group was treated with endoscopic surgery.Pulmonary function,perioperative indicators and complications occurred before and after surgery in both groups.Results The FEVl/FVC of the observation group was(79.52±6.85)%,which was higher than that of the control group(70.12±6.23)%,FVC was(2.75±0.59)L,higher than that of the control group(2.45±0.35)L,FEVl(2.99±0.89)L,higher than the control group of(2.71±0.26)L.The difference was statistically significant(t=8.457,9.637,10.637,P<0.05).The drainage time was shorter,the intraoperative blood loss was less,and the difference was statistically significant(t=5.669,9.669,12.529,P<0.05).The complication rate of the observation group was 6.25%,which was lower than that of the control group(32.26%).The difference was statistically significant(χ^2=6.907,P<0.05).Conclusion The clinical efficacy of laparoscopic surgery in elderly patients with lung cancer is significant,which is helpful to improve postoperative lung function.
