经尿道前列腺电切时膀胱造瘘与非造瘘对术中麻醉管理血钠的影响

Effect of Bladder Fistula and Non-ostomy on Intraoperative Anesthesia Management of Blood Sodium During Transurethral Resection of Prostate

目的探析经尿道前列腺电切时膀胱造瘘与非造瘘对术中麻醉管理血钠的影响。方法随机抽取2014年7月—2019年2月该院泌尿外科收治的前列腺增生症患者80例,均行经尿道前列腺电切手术,随机分为对照组40例:不实施膀胱造瘘,观察组40例:实施膀胱造瘘,比较两组血清钠离子浓度变化。结果术前,两组血钠浓度比较差异无统计学意义(P>0.05);术中,观察组患者血清钠浓度为(139.37±3.56)mmol/L高于对照组,比较差异有统计学意义(t=6.734,P<0.05);观察组术中血钠降低发生率2.50%低于对照组15.00%,差异有统计学意义(χ^2=8.304,P<0.05)。结论膀胱造瘘可以有效降低经尿道前列腺电切手术的前列腺增生症患者的血清钠离子浓度,缓解术中并发症的发生,特别是TURP综合征的发生,有助于预后康复,值得临床推广应用。

Objective To investigate the effect of bladder fistula and non-surgical treatment on intraoperative anesthesia management of blood sodium during transurethral resection of prostate.Methods A total of 80 patients with benign prostatic hyperplasia admitted to the hospital from July 2014 to Feburary 2019 were random enrolled in the Department of Urology.All patients underwent transurethral resection of the prostate.They were randomly divided into the control group(40 cases):no bladder fistula,observation group 40 cases:the bladder fistula was performed and the changes of serum sodium ion concentration were compared between the two groups.Results Before operation in serum sodium concentration between the two groups,there was no statistically significant significant difference(P>0.05).During the operation,the serum sodium concentration of the observation group was(139.37±3.56)mmol/L higher than that of the control group,and the difference was statistically significant(t=6.734,P<0.05);the incidence of blood sodium reduction in the observation group was 2.50%lower than that in the control group15.00%,the difference was statistically significant(χ^2=8.304,P<0.05).Conclusion Bladder ostomy can effectively reduce the serum sodium ion concentration in patients with benign prostatic hyperplasia undergoing transurethral resection of the prostate,relieve the occurrence of intraoperative complications,especially the occurrence of TURP syndrome,which is helpful for prognosis and rehabilitation.It is worthy of clinical application.
