外伤性颅内出血患者预后相关因素研究

Analysis of factors related to prognosis of traumatic intracranial hemorrhage

目的:探讨外伤性颅内出血患者预后的相关性因素。方法:回顾性研究52例颅脑外伤手术患者的CT报告及其他病例资料,观察外伤性颅内出血患者临床基本信息(包括性别、年龄、外伤原因、出血量、出血类型、入院症状、外伤至入院间隔时长、入院时血清电解质水平、术后并发症等),根据出院随访所得mRs评分分组,用多因素logistic回归分析研究各因素与预后相关性。结果:52例手术患者中,20例(38.4%)预后较好。根据logistic回归分析可以得出,在各类因素中,年龄、出血量、出血类型、外伤至入院间隔时长、入院时血清电解质水平等对预后影响较为关键。结论:外伤性颅内出血患者病情复杂,治疗难度较大,有诸多复杂的预后相关性因素,在治疗过程中,应当加强对患者的监护;血清钙离子水平,外伤至入院间隔时长等可以在一定程度上预测患者的预后,对于高危患者,应当及时采取有效的治疗方式,从而提高患者预后水平及生活质量。

Objective:To analyze the correlation between the characteristics of patients with traumatic intracranial hemorrhage and their prognosis.Methods:A retrospective study of case data and CT reports of 52 patients diagnosed with traumatic intracranial hemorrhage and treated with operation were performed.We collected their basic clinical information,including gender,age,cause of trauma,bleeding volume and type of bleeding,admission symptoms,admission time(from incidence to admission),serum electrolyte levels at admission,postoperative complications,etc.Patients were grouped based on mRs score calculated from follow-up,and multivariate logistic regression analysis was used to study the correlations between factors and prognosis.Results:Among 52 patients treated surgically,20 patients(38.4%)had a good prognosis.Among various factors,age,bleeding volume,type of bleeding,admission time and serum electrolyte levels at admission were of great importance for prognosis.Conclusion:Complex conditions of patients with traumatic intracranial hemorrhage have made them difficult to be cured.However,there are many complicated prognostic factors that should be paid more attention to during the treatment.And according to our results,serum calcium level and admission time as well as other factors,in a certain extent,can be potential factors that may predict the prognosis of patients.For patients with these risk factors,effective treatment should be taken in time not only to improve the prognosis but also prompt the quality of life in patients.
