养阴清肺汤加味联合西药治疗慢性阻塞性肺疾病稳定期的临床疗效观察

Yangyin Qingfei decoction combined with western medicine in the treatment of stable chronic obstructive pulmonary disease

目的探讨养阴清肺汤加味联合西药治疗慢性阻塞性肺疾病稳定期的效果,为治疗COPD提供依据。方法选取2018年9月~2019年3月曲靖市中医医院肺病科门诊COPD患者60例,随机分为观察组和对照组,对照组予支气管扩张剂及糖皮质激素治疗,观察组在对照组基础上同时给予养阴清肺汤加味治疗,观察两组患者病情变化及相关指标的变化。结果治疗前,两组患者中医症候评分、肺功能指标、CAT评分及6MWT积分比较,差异无统计学意义(P>0.05),治疗后两组患者中医症候评分、CAT评分及6MWT均较前改善;且观察组优于对照组(P<0.01)。治疗后观察组肺功能指标FVC指标改善不明显(P>0.05),但FEV1%较治疗前改善(P<0.05),且FEV1优于对照组(P<0.05)。结论养阴清肺汤加味联合西药治疗慢性阻塞性肺疾病稳定期的疗效显著,可改善患者生存预后,且无毒副作用,疗效确切,安全性好,体现中西医结合治疗优势,值得临床推广。

Objective To observe the effect of modified Yangyin Qingfei decoction combined with Western medicine in the treatment of stable chronic obstructive pulmonary disease(COPD),and to provide evidence for the treatment of COPD.Methods 60 Patients with COPD from the department of pulmonary diseases in Qujing city hospital of TCM from September 2018 to March 2019 were selected and randomLy divided into observation group and control group.The control group was given bronchodilators and glucocorticoids.The observation group was given the treatment of modified Yangyin Qingfei Decoction on the basis of the control group.Changes in the disease conditions and changes in related indicators were observed in both groups of patients.Results Before treatment,there was no statistically significant difference in TCM symptom score,lung function index,CAT score and 6 MWT between the two groups(P>0.05).After treatment,the TCM symptom score,CAT score and 6MWT in the two groups were improved;the observation group was better than the control group(P<0.01).After treatment,the FVC index of the lung function in the observation group was not significantly improved(P>0.05),but the FEV1%was improved compared with that before treatment(P<0.05),and FEV1%was better than the control group(P<0.05).Conclusion Modified Yangyin Qingfei decoction combined with Western medicine has a significant efficacy in the treatment of stable chronic obstructive pulmonary disease,and improves the survival prognosis of patients.It has no toxic and adverse effects,the curative effect is exact,and the safety is good,reflecting the advantages of integrated Chinese and Western medicine,which is worthy of clinical promotion.
