摘要 目的:观察分析定量磁化率成像(QSM)方法对多发性硬化(MS)患者病灶的检测效能。方法:收集26例MS患者的高分辨T2WI及QSM数据,使用Spearman相关分析比较QSM对每个患者的病灶检出率与患者病程、复发次数之间的相关性;高分辨T2及QSM中大于及小于10mm的病灶数目之间的差异性分析采用卡方检验。结果:高分辨T2W I中观测到病灶891个,其中大于10mm的病灶占36.9%;QSM中观测到病灶727个,大于10mm的病灶占40.4%。QSM序列显示的病灶中均匀稍高信号的病灶519个,其余为环形高信号。QSM对病灶的检出率、病灶平均磁化率与患者的病程及复发次数之间均没有相关性。卡方检验显示高分辨T2WI及QSM两个序列中大于及小于10mm的病灶数目之间的差异性没有统计学意义。结论:QSM对MS颅内病灶有较高的检出率,同时能对MS病灶铁含量进行定量检测,其联合高分辨T2WI能更好的理解病灶特征,提高病灶检测的特异性。 Objective To observe and analyze the efficacy of quantitative susceptibility mapping(QSM)in the detection of lesions in patients with multiple sclerosis(MS).Methods Data of high resolution T2WI and QSM in 26 MS patients were collected,the Spearman correlation analysis was used to compare the correlation between the lesion detection rate of each patient and the disease course and recurrence frequency of each patient.Chi-square test was used to analyze the difference between the number of lesions greater than and less than 10mm in high resolution T2WI and QSM.Results In high-resolution T2WI,891 lesions were observed,among which 36.9%were lesions larger than 10mm.In QSM,727 lesions were observed,and lesions larger than 10mm accounted for 40.4%.The QSM sequence showed 519 lesions with uniform and slightly high signal,and the rest with circular high signal.There was no correlation between the detection rate of lesions and the average susceptibility of lesions by QSM and the course of disease and recurrence frequency of patients.Chi-square test showed no statistically significant difference in the number of lesions greater than or less than 10mm in the high-resolution T2WI and QSM sequences.Conclusion QSM has a high detection rate for intracranial lesions of MS,and can conduct quantitative detection of iron content in lesions of MS.Combined with high-resolution T2WI,it can better understand the characteristics of lesions and improve the specificity of detection of lesions.
